摘　　要：高血压是常见多发性心脑血管病,是生活方式病。契约式管理模式是管理、监控、治疗高血压等慢性病的可行模式。通过契约模式管理高血压,能够优化血压管理,早诊早治和规范治疗,改变生活方式,提高高血压的知晓率、治疗率和控制率,减少心脑血管急性事件,降低医疗消耗,提高生命质量。Hypertension is a common multiple cardio-cerebrovascular disease,the way of life of disease. Contract management mode is a feasible mode for management,monitoring and treatment of hypertension and other chronic diseases. Contract management of hypertension can optimize the management of hypertension,early diagnose and treat,change life style,improve the awareness rate,treatment rate and control rate of hypertension,reduce cardiovascular acute events,reduce consumption of medical and improve the quality of life.
