Influence of Banong Paste on Bacteriological and Morphological Changes of Wound Surface in Patients with Early Soft-tissue Infection

摘  要:【目的】探讨拔脓膏配合抗生素对软组织感染早期创面细菌学和形态学的影响。【方法】选择软组织损伤感染早期创面患者120例,其中大肠埃希菌、鲍曼不动杆菌、铜绿假单胞杆菌、金黄色葡萄球菌感染创面各30例。按照感染创面的细菌种类,采用分层随机化方法将患者随机分为试验组和对照组各60例。试验组采用常规方法清创消毒创面,用拔脓膏填敷创面覆盖无菌纱布,同时配合敏感抗生素治疗;对照组采用常规方法清创消毒创面,外敷优锁纱,同时配合敏感抗生素治疗。2组均每天换药1次,疗程2周。每次换药时评估创面情况,并观察治疗前和治疗2周后感染创面的细菌学和形态学指标变化。【结果】(1)治疗2周后,试验组的大肠埃希菌、鲍曼不动杆菌、铜绿假单胞杆菌、金黄色葡萄球菌的阳性率分别为6.25%、14.29%、6.67%、0.00%,对照组分别为35.71%、50.00%、40.00%、0.00%。试验组的大肠埃希菌、鲍曼不动杆菌、铜绿假单胞杆菌的阳性率显著低于对照组,差异有统计学意义(P<0.05)。(2)试验组的肉芽生长情况优于对照组,差异有统计学意义(P<0.05)。(3)治疗过程中,2组患者的血、尿、大便常规及心、肝、肾功能均无异常变化,也未见局部过敏反应或其他不良反应。【结论】拔脓膏具有显著的抑菌杀菌效果,并能显著促进软组织感染创面的愈合。Objective To investigate the effect of Banong Paste combined with antibiotics on bacteriology and morphology of wound surface in early soft-tissue infection patients. Methods One hundred and twenty qualified patients were enrolled into the study. In 120 patients, 30 had wound surface infection with Escherichia coli, 30 had infection with Acinetobacter baumannii, 30 had infection with Pseudomonas aeruginosa, and 30 had infection with Staphylococcus aureus. With reference to the age, sex and bacterial species, the patients were randomly divided into experiment group and control group, 60 in each group. Both groups were given conventional debridement and wound surface disinfection, and also given intravenous drip of sensitive antibiotics. Moreover, the wound surface of the experiment group was covered with sterile gauze besmeared with Banong Paste, while that of the control group was covered with boric acid solution sterile gauze. Both groups had dressing change once a day and the treatment covered 2 weeks. The wound face was examined during the dressing change, and the bacteriological and morphological changes of the wound surface were compared before treatment and after treatment for 2 weeks. Results (1) After treatment for 2 weeks, the positive rate of Escherichia coli, Acinetobacter baumannii, Pseudomonas aeruginosa and Staphylococcus aureus in the experiment group was 6.25%, 14.29%, 6.67%, 0.00%, and was 35.71%, 50.00%, 40.00%, 0.00% in the control group, respectively. The experiment group had lower positive rate of Escherichia coli, Acinetobacter baumannii, and Pseudomonas aeruginosa (P〈0.05 compared with that in the control group) . (2) The hyperplasia of granulation tissue in experiment group was superior to that in the control group (P 〈0.05) . (3) During the treatment, the routine parameters of blood, urine and stool as well as the function of heart, liver and kidney were steady, and no focal allergic reaction or other adverse reaction was found.
