摘　　要：目的探讨肝吸虫病不同临床时期的超声表现。方法回顾性分析128例临床确诊为肝吸虫病患者的超声检查资料,对比分析不同临床时期(隐性期、发病期、稳定期)的超声表现特征。结果隐性期仅表现为Ⅱ、Ⅲ级胆管或末梢胆管轻度扩张,可合并有胆囊轻度增大;发病期组Ⅱ、Ⅲ级胆管呈"柱状"及"软藤样"扩张,末梢胆管呈"囊状"扩张表现,肝内外胆管扩张程度不一致,管壁毛糙且回声增强,37%病例胆囊增大呈吹气球样改变,34%病例囊内见飘絮物,17.4%病例合并阳性结石;稳定组肝内胆管扩张程度相对发病组轻,但管壁僵直,而回声与发病组相似,胆囊相对发病组小,部分病例囊壁毛糙或均匀增厚,囊内阳性结石发生率明显高于发病组。结论掌握肝吸虫病各期超声表现特征,对肝吸虫病的筛查、诊断、临床疗效评估及愈后随访具有重要意义。Objective To observe the ultrasonic manifestations of clonorchiasis sinensis at different clinical stages.Methods The ultrasonographic data of 128 patients with clonorchiasis were retrospectively analyzed.Ultrasonographic features were compared at different clinical stages（implicit,disease and stable stages）.Results At the implicit stage,there were mild expansion in level 2 and level 3 bile ducts or distal bile duct and a mild increase of gallbladder.At the disease stage,level 2 and level 3 bile ducts showed ＂ columnar＂ and ＂ soft rattan like＂ expansion and peripheral bile duct showed ＂ vesicular＂ expansion.The expansion extent of intrahepatic and extrahepatic bile ducts was not consistent.The tube wall appeared crude and increased echo.37% of gallbladder increased like blowing up a balloon,34% of gallbladder appeared drifting things,and 17.4% of gallbladder combined positive calculi.At the stable stage,intrahepatic bile duct dilatation was milder than that at the disease stage.However,the tube wall was stiffness and the echo was similar to that at the disease stage.Gallbladder was smaller than that at the disease stage.Part of gallbladder wall appeared crude or uniform thickness.The rate of gallbladder calculi at the stable stage was significantly higher than that at the disease stage.Conclusion its important significance to understand the ultrasonographic features of clonorchiasis sinensis for screening,diagnosis,clinical curative effect evaluation and follow-up after healing.
