电子交叉配血技术中供血者不规则抗体筛查的临床研究

摘　　要：目的探讨供血者不规则抗体筛查在电子交叉配血技术安全性方面的重要作用。方法对2011年3月至2012年12月遂宁市中心血站提供的14746袋红细胞成分进行不规则抗体筛查,结果阳性者作抗体特异性鉴定,阴性者按电子交叉配血规则与受血者进行电子交叉配血,同时采用凝聚胺法对供受血者进行传统血清学交叉配血平行试验,分析供血者不规则抗体漏检情况。结果 14746份血液筛查出不规则抗体阳性12例,阳性率0.08%;电子交叉配血均未发现不相容情况;传统血清学交叉配血平行试验未发现次侧不相容者。13215袋供血者红细胞成分发出,无溶血性输血不良反应发生,未发现供血者不规则抗体漏检。结论虽然供血者不规则抗体阳性率低,但临床常用抗体筛查试剂红细胞涵盖抗原谱不广泛,可能造成某些低频率抗体漏检,及时制定不规则抗体筛查试剂红细胞标准方可确保电子交叉配血技术的安全应用。Objective To explore the important role of donor irregular antibody screening in the security of electronic crossmatching technology.Methods Irregular antibody screening was performed in red blood cells components of 14746 samples provided by the Suining Blood Center from March,2011 to December,2012.Specific antibodies were identified in the samples with positive results while negative samples were used to perform electronic cross matching with recipient blood according to rules of this technique.At the same time,parallel tests by using Polybrene method of traditional serological blood cross matching for recipients and donors were performed to investigate the situation of missing detection of irregular antibodies.Results Irregular antibody screening positive was found in 12 cases among the 14746 donor blood samples.The positive rate was 0.08%.No incompatible situation was found by using electronic cross matching technique.No incompatible situation was found by using Polybrene method.There were 13215 donor red blood cell component issued.No hemolytic transfusion adverse reactions occurred and no missing detection of donor irregular antibody.Conclusion Although the positive rate of irregular antibody in donor blood is low,some incidences of antibody missing detection may occur due to that red blood cell antigen spectrum are not widely.The timely development of RBC standards of irregular antibody screening reagent can ensure the safe application of electronic cross-matching technique.
