头低俯卧位支具应用于玻璃体切除联合硅油填充术后患者的效果观察

摘　　要：目的观察头低俯卧位支具减轻玻璃体切除联合硅油填充术后患者被动体位痛苦,减少术后并发症的效果。方法将264例玻璃体切除联合硅油填充术患者按随机数字表法分为对照组和观察组各132例,对照组采取常规俯卧位维持体位,观察组使用头低俯卧位支具保持体位,比较两组一天俯卧位维持时间、首次睡眠时间、每晚睡眠总时间、体位相关不适症状及术后一月并发症的发生。结果观察组一天俯卧位维持时间、首次睡眠时间、每晚睡眠总时间均长于观察组,体位相关不适症状及术后一月并发症的发生均低于对照组(P<0.05,P<0.01),差异有统计学意义。结论头低俯卧位支具的应用可增加玻璃体切除联合硅油填充术后患者的体位维持时间,增加睡眠时间,降低体位相关不适及减少手术并发症的发生。Objective To observe the effect of application of head-lowered prone position orthosis in patients after pars plana vitrectomy（PPV） combined with silicon oil/gas filled to alleviate the distress caused by passive position and to decrease the occurrence of postoperative complications.Methods Two hundred and sixty-four cases undergone PPV combined with silicon oil/gas filled were randomly divided into two groups：control and observation groups,132 cases in each group.Convention prone position was adopted to maintain body position in the control group while orthosis prone position was used in the observation group.Hold time of prone position,duration of the first time to fall asleep,hours of sleep every night,discomfort symptoms related to the position in a day and complications in one month postoperation were compared.Results The hold time of prone position,the duration of the first time to fall asleep,the hours of sleep every night were longer in the observation group than in the control group.However,the body-position-related complaint symptoms and the postoperative complications were less in the observation group than that in the control group（P 〈0.05 and P 〈0.01）.Conclusion The application of the head-lowered prone position orthosis may increase the hold time of prone position,the duration of the first time to fall asleep and the hours of sleep every night,but decrease the body-position-related complaint symptoms and the postoperative complications.
