口腔种植手术医院感染隐患分析

摘　　要：目的对口腔种植术过程中及术后护理中的感染进行分析,以提高手术成功率。方法抽取2009年5月-2013年5月经实施过口腔种植术并发生感染的患者67例作为试验组,同期再抽取67名未行口腔种植术的健康者作为对照组;分析试验组的感染类型,并通过细菌的采样与培养,利用美国ATB全自动细菌鉴定测试仪对试验组和对照组的细菌检出率进行测定,以探讨手术室及医院可能发生感染的隐患。结果 67例感染患者中手术伤口感染40例占59.7%,上呼吸道感染27例占40.3%;试验组厌氧菌检出率56.7%,健康组中同种自然牙厌氧菌检出率23.9%,两组结果差异有统计学意义(P<0.01)。结论以厌氧菌为主的细菌更易感染种植的义齿,对口腔种植手术室的环境及器械应采取严格消毒灭菌的措施,医护人员的技能和素质也应得到加强,积极预防和控制感染是种植术能够成功的保障。OBJECTIVE To analyze the infections that occured during the oral implant or in the postoperative nurs- ing so as to raise the success rate of surgery. METHODS From May 2009 to May 2013, a total of 67 patients who underwent the denture implant and were complicated with infections were extracted as the study group, mean- while, 67 healthy people who did not undergo the oral implant were assigned as the control group, then the types of infections in the study group were observed, the bacteria were sampled and cultured, the isolation rates of the bacteria were determined by using the ATB automatic bacteria identification system of US, and the risks of infec- tions in the operating room and hospital were explored. RESULTS Of the 67 patients with infections, there were 40 （59.7 %） cases of surgical wound infections and 27 （40.3 %） cases of upper respiratory tract infections. The isola- tion rate of anaerobic bacteria was 56.7% in the study group, 23.9% in the control group, and there was signifi- cant difference between the two groups （P〈0.01）. CONCLUSION The implanted dentures are vulnerable to be in- fected with the anaerobic bacteria. It is necessary to take rigid disinfection and sterilization measures for the oral implant operation room as well as the instruments and intensify the skills and qualities of the health care workers so as to effectively control the infections and ensure the success of oral implant.
