摘 要:目的探讨人乳头状瘤病毒(HPV)感染对宫颈癌局部免疫功能的影响,为宫颈癌的预防和治疗方案的选择提供理论依据。方法选取医院2004年8月-2010年8月收治并确诊HPV感染的283例女性患者,按照疾病类型分为宫颈癌组109例及宫颈上皮内瘤变(CIN)组174例,对比不同疾病患者的宫颈局部免疫功能差异;所有数据采用SPSS13.0进行分析。结果宫颈癌组白细胞计数明显低于CIN组,其免疫球蛋白(lg)IgG、IgM及IgA明显低于CIN组及正常标准,CIN组Ig水平均显著高于正常标准,两组患者红细胞计数差异无统计学意义;肿瘤坏死因子-α(TNF-α)、干扰素-γ(INF-γ)、白细胞介素-6(IL-6)及IL-10水平宫颈癌组均显著低于CIN组,宫颈癌组分别为(24.9±8.3)ng/L、(3.8±2.2)μg/L、(5.5±1.9)ng/L及(7.2±2.4)ng/L,CIN组分别为(85.3±193.7)ng/L、(6.6±4.1)μg/L、(18.3±4.7)ng/L及(17.9±9.2)ng/L。结论持续HPV感染导致免疫机能出现下降,导致局部免疫功能耗竭的状况出现,是影响其生存质量的主要原因。OBJECTIVE To explore the impact of human papilloma virus (HPV) infection on the local immune functions of patients with cervical cancer so as to provide theoretical bases for prevention and treatment of cervical cancer. METHODS A total of 283 female patients with confirmed HPV infection, who were treated in the hospital from Aug 2004 to Aug 2010, were enrolled in the study and divided into the cervical cancer group with 109 cases and the cervical intraepithelial neoplasia (CIN) group with 174 cases according to the types of diseases, then the local cervical immune functions were observed and compared between the two groups, and all the data were statis- tically analyzed with the use of SPSS13.0 software. RESULTS The white blood cells counts of the cervical cancer group were significantly less than those of the CIN group; the levels of the immunoglobulin (Ig) G, IgM, and IgA of the cervical cancer group were significantly lower than those of the CIN group; the levels of Ig of the CIN group were significantly higher than the normal standard; there was no statistically significant difference in the red blood cells counts between the two groups. The levels of the tumor necrosis factor-a(TNF-a), interferon-y(INF-7), interleukin-6(IL-6), and IL-6 of the cervical cancer group were respectively (24.9±8.3)ng/L, (3.8±2.2)μg/L, (5.5±1.9)ng/L, and(7.2±2.4)ng/L, significantly lower than(85.3±193.7)ng/L, (6.6±4.1)μg/L, (i8.3± 4.7)ng/L, and (17.9±9.2)ng/L of the CIN group. CONCLUSION The continuous HPV infection may result in the declined immune functions and the exhaustion of local immune functions, which is a leading influencing factor for the quality of life.
