摘 要:目的探讨小儿肺炎衣原体感染与心肌炎临床关系,并为心肌炎的临床防治提供经验积累。方法选取医院于2010年4月-2013年3月收治的60例心肌炎患儿作为研究对象,患儿在入院24h后及入院后第3周采集静脉血,采取微量免疫荧光方法检测血清中特异性IgG、IgM;采用数据录入SPSS 19.0软件进行统计分析。结果 60例患儿肺炎衣原体感染17例,其中IgG阳性4例、IgM阳性5例、IgG强阳性8例,阳性率28.3%;肺炎衣原体阳性患儿呼吸道感染史、胸闷、气短的发生率分别为100.0%、88.2%、76.5%,均高于肺炎衣原体阴性患儿的51.2%、79.1%、67.4%,两组之间差异有统计学意义(P<0.05);肺炎衣原体阳性患儿经心肌营养治疗后,10例患儿的临床症状得到改善,7例患儿临床症状尚未完全消失或者实验室检查结果中仍存在≥2项异常。结论小儿肺炎衣原体感染与心肌炎存在一定程度的关联性,在临床中加强肺炎衣原体感染特异性抗体监测具有重大临床意义。OBJECTIVE To investigate the clinical relationship between Chlamydia pneumoniae infection and myo- earditis in children so as to provide experience for the prevention and treatment of myocarditis in clinical practice. METHODS A total of 60 cases of myocarditis children treated in the hospital from April 2010 to March 2013 were selected as study subjects; the venous blood of those children were collected 24 hours later and 3 weeks later after their admission of hospital, and micro immunofluorescence assay was adopted for testing the specific IgG, and IgM in serum; data entry SPSS 19.0 software was adopted for statistical analysis. RESULTS Totally, 17 of the 60 myo- carditis patients suffered Chlamydia pneumonia (C. pneumonia) infection, thereinto, 4 cases presented positive IgG, 5 cases presented positive IgM, and 8 cases presented strong positive IgG; the positive rate was 28.3% ; the incidence rates of respiratory tract infection, chocking sensation in chest and shortness of breath for children suffering positive C. pneumonia were 100.0%, 88.2% and 76.5% respectively, higher than 51.2%, 79.1% and 67.4% for children suffering negative C. pneumonia; there was significant difference between the two groups of children (P〈0. 05) ;after receiving myocardial nutrient, 10 cases of children with positive C. pneumonia got clini- cal symptoms improved, 7 cases of children did not get the clinical symptoms completely disappeared or still had 2 or more abnormal items in the laboratory results. CONCLUSION There is certain relationship between C. pneumo- nia infection and myocarditis, so it is of great clinical significance to strengthen the monitoring of C. pneumonia specific antibody in clinical practice.
