黄地肾衰瘙痒洗液治疗尿毒症皮肤瘙痒34例临床观察

摘　　要：目的观察黄地肾衰瘙痒洗液治疗尿毒症皮肤瘙痒的临床疗效。方法将70例尿毒症皮肤瘙痒患者随机分为治疗组和对照组各35例,两组均给予规律血液透析(每周2或3次,每次4h)等基础治疗,对照组在基础治疗之外口服氯雷他定片10mg,每日1次。治疗组在基础治疗之外,用黄地肾衰瘙痒洗液洗浴,每日1次。两组均治疗15天。观察治疗前后瘙痒积分、血肌酐、尿素氮、血钙、血磷变化,治疗后评价临床疗效。结果治疗组临床疗效总有效率为91.18%,对照组为70.59%,治疗组明显优于对照组(χ2=4.660,P=0.031)。两组患者治疗后瘙痒积分均较治疗前有明显下降(P<0.01),且治疗组瘙痒积分下降明显优于对照组(P<0.05)。治疗组治疗后血肌酐、尿素氮、血钙、血磷均明显改善(P<0.05);治疗组治疗后血肌酐、尿素氮、血磷水平明显低于对照组(P<0.05)。结论黄地肾衰瘙痒洗液能明显缓解尿毒症皮肤瘙痒症状,改善肾功能,临床疗效优于氯雷他定片。Objective To observe the efficacy of Huangdi Shenshuai Saoyang Lotion （Lotion for uremic pruritus,黄地肾衰瘙痒洗液） for uremic pruritus of. Methods Seventy cases of uremic pruritus were randomized into the treatment group and control group, with 35 in each. Both groups were given basic treatment such as hemodialysis （2 or 3 times weekly and 4h once）. The patients in the control group were orally taken 10 mg Loratadine Tablets once daily. The patients in the treatment group were bathed with Huangdi Shenshuai Saoyang Lotion once daily. The treatment lasted for 15 days. The changes of itching points, serum creatinine （Scr）, blood urea nitrogen （BUN）, calcium （Ca） and phosphorus （P） were observed before and after treatment. The efficacy was evaluated. Results The total effective rate of the treatment group and control group was 91. 18% and 70. 59% respectively, with a significant difference between groups （ χ^2 = 4. 660, P = 0. 031 ）. The itching points were significantly decreased in both groups after treatment （ P 〈 0. 01 ）, especially in the treatment group （P 〈 0. 05 ）. The levels of Ser, BUN and P were significantly decreased but the level of Ca was significantly increased in the treatment group after treatment （ P 〈 0.05 ）. After treatment, the levels of Scr, BUN and P in the treatment group were significantly lower than those in the control group （P 〈 0.05 ）. Conclusion Huangdi Shenshuai Saoyang Lotion can obviously relieve the symptoms of uremic pruritus and improve renal function, with a better efficacy as compared with Loratadine Tablets.
