摘 要:目的初步探讨热毒宁注射液过敏反应的可能机理。方法采用嵌套式巢式病例对照研究的方法,对使用热毒宁注射液的3000例感染性疾病患者进行监测,对出现过敏反应的患者按1∶4比例匹配,匹配因素为性别、年龄±5岁、用药季节、药品批次等,另取4例健康志愿者,检测免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、血清总补体C3(C3)、血清总补体C4(C4)、白细胞介素-2(IL-2)、白细胞介素-4(IL-4)、白细胞介素-6(IL-6)、白细胞介素-10(IL-10)、重组人干扰素γ(IFN-γ)、总免疫球蛋白IgE(TIgE)。结果 3000例使用热毒宁注射液患者中有2例出现过敏反应,根据匹配原则共检测血清样本14例,2例过敏患者TIgE均明显高于正常范围,IL-10、IL-4均升高,IgA、IgG、C3、C4、IL-2、IFN-γ均在正常范围内与健康志愿者及匹配患者相比无显著差别,提示患者机体的免疫应答系统激活。结论热毒宁注射液过敏反应可能为由IgE介导的Ⅰ型过敏反应。Objective To study the possible mechanism of allergic reactions of Reduning Injection. Methods An embedded nested case-control study was conducted. Totally 3 000 patients with infectious diseases used Reduning Injection were monitored. The allergic patients were matched at a ratio of 1 to 4 by taking gender, age + 5 years, medication season and drug batch as the matching factors. Four healthy volunteers were selected. The levels of immunoglobulin A ( IgA), immunoglobulin G (IgG), complement 3 ( C3 ), complement 4 ( C4), interleukin-2 ( IL-2 ), interleukin-4 (IL-4), interleukin-6 (IL-6), interleukin-10 (IL-10), interferon-g (IFN-γ) and total immunoglobulin E (TIgE) were detected. Results There were 2 allergic patients. Fourteen serum samples were detected according to the matching principle. The level of TIgE in 2 allergic patients was significantly higher than the normal. The levels of IL-10 and IL-4 in 2 allergic patients were increased. The levels of IgA, IgG, C3, C4, IL-2 and IFN-γ were within the normal, with no significant difference between the healthy volunteers and matched patients. The results showed that the immune response system was activated in the patients. Conclusion The allergic reactions of Reduning Injection may be IgE-mediated type I allergic reactions.
