二级医院效率及影响因素的实证研究

摘　　要：目的:系统研究某地14家二级综合医院的运行效率及影响因素,探讨提高医院运行效率的方式和途径。方法:采用DEA-C2R与BC2模型计算14家医院的整体效率、纯技术效率和规模效率,再采用多元线性回归模型对影响效率的相关因素进行分析。结果:14家医院中有4家医院(28.6%)的整体效率为1,处于固定规模报酬状态。10家医院(71.4%)的整体效率小于1,其中8家医院处于规模报酬递减阶段。人均GDP、财政补助收入对纯技术效率的影响具有统计学意义(P<0.05);实际开放床位数、常住人口数和平均住院日对规模效率的影响具有统计学意义(P<0.05);常住人口数,财政补助收入和每职工年均门急诊人次对整体效率的影响具有统计学意义(P<0.05)。结论:通过改善内部管理水平,缩短平均住院日,提高每职工年均门急诊人次等方式可以提高样本医院的运行效率。Objective：To analyze the operational efficiency of 14 level two general hospitals of a region and the influencing factors, discuss the method of improving hospital operation efficiency. Methods：Using DEA-C2R and BC2 model to calculate the overall efficiency, pure technical efficiency and the scale efficiency, then analyze the related factors multi-liner regression model on influencing efficiency. Results： The whole efficiency of d hospitals （28.6%） out of 14 hospitals was below 1. which were at the fixed scale repayment stage. The whole efficiency of 10 hospitals （71.4%） was below 1.8 hospitals were at scale repayment decreasing stage. The influences of per capital GDP and financial？subsidies on pure technical efficiency has statistical influences （P〈0.05）; the influences of actual open bed number, permanent resident population and actual average hospital stay on scale efficiency has statistical influences （P〈0.05） ; the influences of permanent resident population, financial？subsidies and per capita employee average emergency visit number on the whole efficiency has statistical influences（P〈0.05）. Conclusion： To promote the operation efficiency of the sample hospital, improving the internal management level, shortening average hospital stay and promoting per capita employee average emergency visit number are needed.
