性别对人工全膝关节置换术后临床疗效的影响

摘　　要：目的探讨性别因素对膝关节骨关节炎患者全膝关节置换术后临床疗效的影响。方法随机选择2008年1月-2012年12月在中南大学湘雅三医院行初次单侧人工全膝关节置换术的100例膝关节骨关节炎患者（男、女各50例）,回顾性分析患者术后1周、2周、1年的临床疗效和术后1年的满意度差异。结果男女患者术后1周膝关节疼痛、ROM无差异（P〉0.05）,男性患者术后2周和术后1年的膝关节疼痛评分、ROM及术后1年的膝关节功能评分、HSS评分较女性好（P〈0.05）;术后1年男女患者满意度显示无差异（P〉0.05）。结论性别因素对膝关节骨关节炎患者全膝关节置换术后膝关节疼痛缓解、膝关节ROM及功能改善有影响;男女患者术后短期满意度无差异。【Objective】To discuss the influence of gender on postoperative clinical effects of knee osteoarthritis patients who underwent total knee arthroplasty（TKA）.【Methods】By retrospectively analyzing 50 males and 50 females, from January 2008 to December 2012 randomly selected from 100 knee osteoarthritis patients who firstly received unilateral TKA in our hospital. Their clinical effects after 1 week, 2 weeks and 1 year were assessed with HSS score, and the degree of satisfaction of male and female patients one year after surgery were assessed with SF-12. Results were statistically analyzed according to t-test and χ^2test. 【Results】There were no statistical difference between male and female patients＇ knee pain 1 week after the operation（P〈0.05）, but the knee pain, range of motion, functions and HSS total score 2 weeks and 1 year after the operation for male patients are better than female patients（P〈0.05）. The satisfaction degree after 1 year for both male and female patients was not significantly different（P〉0.05）.【Conclusions】Gender has some influence on knee pain relief, knee ROM and knee functions of knee osteoarthritis patients after TKA. The short-term satisfaction degree between male and female patients has no statistical difference.
