CSEA与TIVA麻醉方式在泌尿科激光碎石手术中的应用效果分析

摘　　要：目的 探讨腰硬联合麻醉（Combined Spinal Epidural Anesthesia,CSEA）与全凭静脉麻醉组（Total Infusion Venus Anesthesia,TIVA）在泌尿外科激光碎石手术中的应用效果分析.方法 回顾性分析本院自2012年1月至2013年8月收治的泌尿系结石患者86例,按照麻醉方式的不同随机分为CSEA组和TI-VA组.对术中术后的相关指标进行分析.结果 两组按照手术时间、结石返回率、进镜困难发生率、体动发生率、患者不适发生率、麻醉费用等进行比较,差异有统计学意义（P＜0.05）;麻醉前及麻醉后30min,CSEA组和TIVA组的平均动脉血压、血氧饱和度及心率进行比较,差异有统计学意义（P＜0.05）;麻醉后24h,CSEA组总满意率（很满意＋满意）为79.07％ （34/43）,TIVA组总满意率为97.67％ （41/43）.两组总满意率比较,差异有统计学意义（P＜0.05）.结论 采用全凭静脉麻醉联合钬激光治疗泌尿系结石具有安全、舒适、疗效显著等优势,是治疗泌尿系结石的一种有效的麻醉方法.Objectives To expore the application effect of urinary laser lithotripsy by CSEA and TIVA anesthesia methods.Methods 86 patients with urinary calculi were retrospective analyzed from January 2012 to August 2013,in accordance with the different methods of anesthesia,they were randomly divided into CSEA group and TIVA group.In the two groups in postoperative related index was analyzed.Intraoperative and postoperative related index of two groups of patients was analyzed.Results According to the operation time,the incidence rate of calculi return,the incidence of the mirror difficult,the incidence of discomfort,anesthetic fees,two groups were compared,the difference was statistically significant （P ＜ 0.05）; Before anesthesia and anesthesia after 30 minutes,mean arterial blood pressure,blood oxygen saturation and heart rate of two group were compared,the difference was statisticallysignificant（P ＜ 0.05） ; 24 hours after anesthesia,overall satisfaction rate of CSEA group was 79.07％ （34/43）,TIVA group was 97.67％ （41/43）.total satisfaction rate of two groups have a comparison,the difference was statistically significant （P ＜ 0.05）.Conclusions For the treatment of urinary calculi by holmium laser in total infusion venus anesthesia.It has the safety,efficacy significantly,comfortable advantages,it is a kind of effective anesthesia in the treatment of urinary calculi.
