2型糖尿病患者血清胰岛素原与载脂蛋白A1、载脂蛋白B关系研究  被引量:1

摘  要:目的:研究2型糖尿病患者中血清胰岛素原(PI)与载脂蛋白A1(Apo-A1)、载脂蛋白B (Apo-B)的关系。方法:将所有受试者83例,以空腹血清PI水平中位数为界值,分为两组,高PI组和对照组,均行空腹口服葡萄糖耐量试验,空腹采血测PI、真胰岛素( TI)、 Apo-A1、 Apo-B、总胆固醇( TC)、三酰甘油( TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、血糖、糖化血红蛋白,糖负荷后2h采血检测TI、 PI、血糖,计算胰岛素抵抗指数,并测量身高、体重、腰围、臀围、血压等指标进行比较分析。结果:在高PI组, Apo-A1、 HDL-C显著降低, Apo-B、 TC、 TG、 LDL-C显著升高, TI、 HOMA-IR也显著升高;直线相关分析显示,空腹血清PI 水平与Apo -A1、Apo-B以及TC、 TG、 LDL-C、 HDL-C均显著相关;2h PI水平与Apo-A1、 Apo-B、 TG、 LDL-C、 HDL-C也显著相关,与TC不相关;进一步多重线性回归分析显示,校正性别、年龄、体质量指数等参数后, PI与Apo-A1、 Apo-B仍显著相关,2h PI、2h TI只与Apo-B相关,与Apo-A1不再相关。结论:2型糖尿病高PI组患者,存在更明显的Apo-A1下降和Apo-B升高,及其它脂质代谢异常,胰岛素原与Apo-A1下降和Apo-B升高密切相关。OBJECTIVE: To evaluate the relationship between proinsulion and Apo -A1, Apo-B in the patients with type 2 di-abetes.METHODS: All the 83 cases were divided into two groups according to their serum proinsulion ( PI) median: higher PI group and control group.OGTT tests were conducted with the subjects.Fasting and 2hPI, true insulin (TI), plasma glucose, Apo-A1, Apo-B, TC, TG, LDL-C, HDL-C, glycosylated hemoglobin (HbA1c) and blood pressure were all measured .Homeo-stasis model assessment insulin resistance index ( HOMA-IR) was calculated .RESULTS: In higher PI group Apo-A1 and HDL-C were lower; Apo-B, TC, TG and LDL-C were higher; TI and HOMA-IR were higher than those in control group; Pearson correlation analysis showed that fasting serum PI was significantly correlated with Apo -A1, Apo-B, TC, TG, LDL-C and HDL-C.2hPI was also significantly correlated with Apo -A1, Apo-B, TG, LDL-C and HDL-C, but not correlated with TC .After adjusting gender , age and BMI , further multivariable linear regression analysis indicated that PI and TI were still significantly correla -ted with Apo-A1 and Apo-B.2h PI and 2h TI were correlated with Apo -B only, not correlated with Apo -A1.CONCLUSION:In higher PI group with type 2 diabetes Apo -A1 is lower and Apo-B is higher, and other abnormal lipid metabolism also exists . True insulion and proinsulion are closely related to this metabolic disorder .
