菖蒲芪丹汤治疗扩张型心肌病临床研究

摘　　要：目的观察菖蒲芪丹汤对扩张型心肌病患者的影响。方法将90例扩张型心肌病患者随机分为2组。对照组45例予西医常规治疗,治疗组45例在对照组治疗基础上加菖蒲芪丹汤,连续应用2个月。检测2组治疗前后心率、呼吸频率、血压、24 h尿量、心胸比例、左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)、左室射血分数(LVEF)及血浆B型脑钠肽(BNP)浓度变化。结果治疗组总有效率高于对照组(P<0.05)。2组治疗后心率、呼吸频率、心胸比例、LVEDD、LVESD、LVEF、收缩压、舒张压及BNP均较本组治疗前明显下降(P<0.01,P<0.05),24 h尿量升高(P<0.01,P<0.05);且2组治疗后心率、呼吸频率、24 h尿量、心胸比例、LVEDD、LVESD、LVEF及BNP组间比较差异有统计学意义(P<0.01,P<0.05)。结论菖蒲芪丹汤能明显改善扩张型心肌病患者心功能,用药安全,临床效果显著。Objective To observe the effect of dilated cardiomyopathy treated by Changpu Qidan decoction.Methods 90 cases of dilated cardiomyopathy were randomly divided into two groups. Patients in control group（ n =45） were treated by conventional western medicine therapy. Patients in treatment group（ n = 45） were treated by Changpu Qidan decoction on the basis of control group treatment. The course was two months in two groups. The changes of heart rate,respiratory rate,blood pressure,24 h urine volume,cardiothoracic ratio,left ventricular end-diastolic diameter（ LVEDD）,left ventricular systolic diameter（ LVESD）,left ventricular ejection fraction（ LVEF）and plasma B- type natriuretic peptide（ BNP） concentration before and after treatment were detected. Results The total effective rate in treatment group was higher than that in control group（ P〈0. 05）. Heart rate,respiratory rate,cardiothoracic ratio,LVEDD,LVESD,LVEF,systolic blood pressure,diastolic blood pressure and BNP concentration after treatment were significantly decreased in two groups（ P 〈0. 01,P 〈 0. 05）. 24 h urine volume was increased after treatment（ P 〈 0. 01,P 〈 0. 05）. There were significant difference between two groups on heart rate,respiratory rate,24 h urine volume,cardiothoracic ratio,LVEDD,LVESD,LVEF and BNP concentration（ P 〈0. 01,P 〈 0. 05）. Conclusion Changpu Qidan decoction can significantly improve heart function in patients with dilated cardiomyopathy,medication safety,and significant clinical effect.
