摘　　要：目的探讨CD4+Th细胞因子对孕激素治疗先兆流产的影响。方法随机抽取深圳市妇幼保健院2010年7月至2011年2月收治的60例先兆流产患者的临床资料进行回顾性分析,本组所有患者均采用孕激素治疗,同时采用酶联免疫吸附试验对先兆流产患者使用孕激素治疗前、后血清中的CD4+Th细胞因子进行测定,深入探讨CD4+Th细胞因子对孕激素治疗先兆流产的影响;并且与同期60例先兆流产妇女未治疗患者和60例正常早孕者的临床资料进行对比评价。结果经检测发现,孕激素治疗先兆流产患者前、后女性Th1/Th2细胞因子平衡性会有变化,由先兆流产时平衡趋向Th1的方向致使用孕激素治疗后平衡趋向Th2的方向变化,并且各组细胞CD4+Th细胞因子均有不同,其中正常妊娠妇女白细胞介素-2(IL-2)、IL-4、IL-10和干扰素-α(TNF-α)分别为(92.78±7.86)、(62.78±7.86)、(144.78±7.86)和(102.78±7.86)ng/L,与先兆流产未治疗组妇女的(223.78±7.86)、(23.78±7.86)、(154.78±7.86)和(26.78±7.86)ng/L,以及孕激素治疗先兆流产组的(93.78±7.86)、(63.78±7.86)、(52.78±8.33)和(103.78±7.86)ng/L比较,差异有统计学意义(P<0.05)。结论在先兆流产妇女的血清学检查中,Th1型细胞因子如:TNF-a、IL-2活性较正常妊娠女性明显增高,而Th2型细胞因子如:IL-10、IL-4活性较正常妊娠女性基本正常或略降低。Objective To investigate the influence of CD4＋Th cytokines on treatment effects of progesterone in threatened abortion.Methods Clinical data were randomly selected and retrospectively analyzed in 60patients with threatened abortion during July 2010and February 2011.All patients were treated with progestogen,and enzyme-linked immunosorbent assay were performed to detect serum CD4＋Th cytokines before and after treatment.A total of 60patients with untreated threatened abortion and 60cases of normal pregnancy were also enrolled.Results In patients with threatened abortion,Th1/Th2cytokine balance could be changed after progesterone therapy.In cases of normal pregnancy,levels of interleukin（IL-2）,IL-4,IL-10and interferon（IFN）-αwere（92.78±7.86）,（62.78±7.86）,（144.78±7.86）and（102.78±7.86）ng/L,respectively,which were statistically different with the（223.78±7.86）,（23.78±7.86）,（154.78±7.86）and（26.78±7.86）ng/L in patients with untreated threatened abortion,and（93.78±7.86）,（63.78±7.86）,（52.78±8.33）and（103.78±7.86）ng/L in patients with threatened abortion after treatment（P〈0.05）.Conclusion Activity levels of Th1cytokines,including IFN-a and IL-2,in patients with threatened abortion could be higher,but those of Th2cytokines,including IL-10and IL-4,could be as the same level or lower,compared with cases with normal pregnancy.
