摘 要:目的评价盆底肌康复治疗女性盆底功能障碍的效果。方法采用PHENIX神经肌肉电刺激治疗仪,对650例女性盆底功能障碍患者进行盆底肌电刺激联合生物反馈治疗和功能锻炼,比较治疗前后盆底肌肌力及功能状态。结果患者压力性尿失禁治疗有效率为88.4%(311/352),阴道壁膨出治疗有效率为90.9%(431/474),轻中度子宫脱垂治疗有效率为62.4%(151/242);盆底肌肌力明显提高,Ⅰ类肌纤维肌力达4级以上者由治疗前12.6%上升到治疗后67.4%,Ⅱ类肌纤维肌力达4级以上者由治疗前13.6%上升到治疗后70.8%,肌力比较差异均有统计学意义(P<0.01);治疗后肌电值优于治疗前[治疗前后分别为(10.30±4.53)μV和(16.58±5.05)μV],差异有统计学意义(P<0.01);下腹疼痛、大便异常、性功能障碍、腰骶酸痛、阴道下坠感等症状均得到显著改善。结论盆底肌康复治疗是一种安全、有效、经济、方便的盆底功能障碍治疗方法。Objective To evaluate the effect of pelvic floor muscle rehabilitation therapy for female pelvic floor dysfunction. Methods A total of 650 patients with pelvic floor dysfunction were selected , PHENIX neuromuscular stimulation therapeutic apparatus imported from France were used to treat these cases. All the patients underwent biofeedback electrical stimulation combined with pelvic floor muscle training. Pelvic floor muscle strength and functional status of all cases were compared before and after treatment. Results The effective rate of stress urinary incontinence was 88.4%(311/352),the effective rate of vaginal wall prolapse was 90.9%(431/474),the effective rate of light degree and midrange uterine prolapse was 62.4%(151/242). The muscle strength of type Ⅰup to grade four increased from 12.6% to 67.4% after treatment ,the muscle strength of type Ⅱup to grade four increased from 13.6% to 70.8% after treatment ,both of the differences had statistical significance (P&lt;0.01). Myoelectric voltage was significantly increased after treatment (P&lt;0.01),which increased from (10.30&#177;4.53) μV to (16.58&#177;5.05) μV. The symptoms of lower abdominal pain,abnormal defecation, sexual dysfunction,pain in the lumbosacral region,vaginal straining feeling were improved obviously. Conclusion Biofeedback electrical stimulation combined with pelvic floor muscle training is a safe , effective,economic and convenient method to treat pelvic floor dysfunction.
