原发性干燥综合征及继发间质性肺病患者的临床特征与血清半乳糖凝集素-3水平的检测

摘　　要：【目的】探讨半乳糖凝集素-3（galectin-3，GAL-3）在原发性干燥综合征（primary Sjgren＇s syndrome， pSS）及其继发间质性肺病（interstitial lung disease，ILD）发病机制中的作用。【方法】比较73例合并及不合并ILD的pSS患者的临床指标，应用ELISA方法检测73例pSS患者及20例正常对照者血清GAL-3水平。【结果】pSS患者血清 GAL-3水平明显高于正常对照组，且差异有显著性（795．1〈79．3）pg/mL ＆（458．0〈97．4） pg/mL，P＜0．05），而 pSS-ILD患者 Gal-3水平显著高于单纯 pSS患者（1047〈110．7）pg/mL ＆（628．6〈72．62）pg/mL，P＜0．01）。此外，pSS-ILD患者免疫球蛋白 IgG、补体C3、纤维蛋白原（FiB）、C反应蛋白（CRP）及血沉水平明显高于单纯 pSS患者且差异均有显著性（P＜0．05或P＜0．01），血清白蛋白（ALB）则明显低于单纯 pSS患者（P＜0．01）。而且 pSS患者 Gal-3水平与CRP、IgG及 FiB呈正相关关系（r＝0．487，P ＜0．001，r＝0．334，P＝0．004，r＝0．482，P＜0．001）。【结论】半乳糖凝集素-3可能参与pSS的免疫炎症损伤及肺间质纤维化的发生发展。[Obj ective]To explore the role of galectin-3 （GAL-3 ）in pathogenesis of primary Sjgren＇s syn-drome（pSS）patients with interstitial lung disease（ILD）.[Methods]Clinical indexes of 73 pSS patients with and without ILD were compared.Enzyme linked immunosorbent assay（ELISA）was used to detect serum levels of GAL-3 in 73 pSS patients and 20 normal controls.[Results]Serum GAL-3 levels in pSS patients were obviously higher than those in normal controls,and there was significant difference（795.1〈79.3pg/mL vs.458.0±97.4 pg/mL,P〈0.05）.Serum GAL-3 levels in pSS-ILD patients were markedly higher than those in simple pSS pa-tients（1047〈110.7 pg/ml vs.628.6〈72.62 pg/ml,P〈0.01）.Furthermore,the levels of IgG,C3,fibrinogen （FiB）,acute phase reactive protein（CRP）and blood sedimentation in pSS-ILD patients were obviously higher than those in simple pSS patients,and there was significant difference（P〈0.05 or P〈0.01）.Serum albumin（ALB） levels in pSS-ILD patients were obviously lower than those in simple pSS patients（P〈0.01）.Serum GAL-3 lev-els in pSS patients were positively correlated with CRP,IgG and FiB（r=0.487,P 〈0.001,r= 0.334,P =0.004,r=0.482,P〈0.001）.[Conclusion]GAL-3 may participate in immune inflammatory injury of pSS and the occurrence and development of ILD.
