柔肝化纤颗粒对肝硬化门静脉高压的影响及可能机制研究

摘　　要：目的探讨柔肝化纤颗粒对肝硬化门静脉高压的影响及可能机制。方法回顾性分析60例肝硬化门静脉高压所致食管胃底静脉曲张患者的临床资料，给予柔肝化纤颗粒进行治疗。比较治疗前后肝静脉自由压（FHVP）、肝静脉楔压（WHVP）、肝静脉压力梯度（HVPG）、血流动力学及心率（HR）、平均动脉压（MAP）的变化情况。结果柔肝化纤颗粒治疗前后肝静脉自由压（FHVP）、脾静脉内径（Dsv）及肠系膜上静脉血流动力学各指标变化相比，差异无统计学意义（P〉O．05），而治疗前后肝静脉楔压（WHVP）、肝静脉压力梯度（HVPG）、静脉主干血流动力学各指标变化、脾静脉流速（Vsv）及流量（Qsv）、心率（HR）、平均动脉压（MAP）相比，差异均具有统计学意义（P〈0．05）。结论柔肝化纤颗粒可显著地降低肝硬化患者脾静脉与门静脉主干的血流速度及血流量，从而减少门静脉的血流灌注，且能显著降低患者肝静脉压力梯度，对食管胃底静脉曲张破裂出血及腹水的防治具有重要的临床价值。Objective To investigate the effect and possible mechanisms of softening fiber particles on portal hypertension. Methods Sixty cases of patients with liver cirrhosis and portal hypertension caused by esophageal varices were analyzed retrospectively,and they were given softening fiber particles for treating. The data of hepatic venous pressure（FHVP）, hepatic venous wedge pressure（WHVP）, hepatic venous pressure gradient（ HVPG）, hemodynamics and heart rate（HR） ,mean arterial pressure（MAP） were compared before and after treatment. Results There were no statistical differences between the data of FHVP,splenic vein diameter（Dsv） and superior mesenteric venous hemodynamic before treatment and those after treatment（P〉0.05） ,but there were statistical differences on the data of W HVP, HVPG, vein blood hemodynamic, splenic vein and flow velocity Vsv Qsv, HR, MAP before and after treatment（（P〈0.05）. Conclusion Softening fiber particles can significantly reduce the splenic vein in patients with cirrhosis and portal vein blood flow velocity and blood flow,thereby reducing the portal venous perfusion, and significantly lower hepatic venous pressure gradient in patients, it has important clinical value for the prevention and treatment of esophageal varices bleeding and ascites.
