经会阴超声在产后压力性尿失禁诊断及康复治疗疗效评价中的应用

摘　　要：目的观察会阴超声对生物反馈电刺激治疗产后女性压力性尿失禁的临床疗效。方法选择医院分娩的产妇80例,将其随机分成治疗组40例和对照组40例,治疗组患者行生物反馈电刺激治疗联合盆底肌锻炼治疗,对照组患者行盆底肌锻炼治疗,并通过会阴超声观察静息状态和Valsalva动作时膀胱颈下降度,尿道膀胱后角,盆膈裂孔面积等。结果治疗组患者的膀胱颈下降度、尿道膀胱后角及盆膈裂孔面积明显小于对照组患者,差异均有统计学意义(P<0.05)。结论生物反馈电刺激联合盆底肌锻炼治疗产后女性压力性尿失禁效果显著,会阴超声(尤其是三维成像技术)观察压力性尿失禁成像清晰,操作简便,可重复检查,经济实用,可在临床上推广应用。Objective To study the application of transperineal ultrasound on diagnoses and rehabilitative treatment of postpartum stress urinary incontinenc.Methods 80 patients were divided into treatment group（40cases）and the control group（40cases）.The treatment group was treated with biofeedback and pelvic floor muscle exercise.The control group was treated with pelvic floor muscle exercise.We observed the degree of decrease in bladder neck,urinary bladder angle and pelvic diaphragm hiatus area in the resting state and Valsalva maneuver by transperineal ultrasound.Results The degree of decrease in bladder neck,urinary bladder angle and the pelvic diaphragm hiatus area in the treatment group were less than that in the control group.There was a significant difference（P〈0.05）.Conclusion The efficacy of postpartum women with SUI cured by biofeedback pelvic floor muscle exercise is outstanding.SUI by transperineal ultrasound can distinctly image and easily operate.
