鼓室灌注联合静脉注射地塞米松治疗突发性耳聋的临床疗效观察

摘　　要：目的观察鼓室灌注联合静脉注射地塞米松治疗突发性耳聋的临床效果。方法将76例(76耳)突发性耳聋患者随机分为A、B两组,各38例,其中A组予静滴地塞米松,辅以鼓室注射地塞米松。B组则单纯按照A组的静脉用药方法。观察两组患者治疗前后平均听阈值变化及两组有效率情况。结果 A组平均听阈治疗前后分别为(70.24±15.28)dB和(51.28±21.26)dB;B组平均听阈治疗前后分别为(70.35±16.27)dB和(60.17±24.32)dB。A组患者平均听阈改善程度明显优于B组(P<0.05)。A组治疗总有效率为94.7%高于B组78.9%(P<0.05)。结论鼓室灌注联合静脉注射地塞米松治疗突发性耳聋可以提高临床疗效,是目前治疗突发性耳聋有效治疗方案。Objective To observe the clinical effect of intratympanic dexamethasone perfusion in the treatment of sudden deafness. Methods Thirty-eight subjects（ thirty-eight ears） were divided into two groups randomly,including group A（ 38 subjects; 38 ears） and group B（ 38 subjects,38 ears）. The patients in treatment group A were given intravenous infusion of dexamethasone,combined with perfusion of dexamethasone into the tympanum. The control group simply applied intravenous infusion of dexamethasone. The efficiency and the average hearing threshold changes of two groups before and after treatment was observed. Results Before and after treatment,the average hearing threshold was（ 70. 24 ± 15. 28） dB and（51.28 ±21.26） dB in the treatment group. And the average hearing threshold was（ 70.35 ±16.27） dB and（ 60.17 ±24.32） dB in the control group. The clinical effect of the group A was significantly better than the group B（ P〈 0. 05）. The effective rate of group A（ 94. 7%） was significantly higher than that of the group B（ 78. 9%）. There was no obvious adverse reaction and complication in two groups. Conclusion The treatment for sudden deafness with dual pathways combined with hormone can improve the clinical efficacy for the treatment of sudden deafness therapy.
