氟马西尼在不同年龄组妇科腔镜患者全麻后清醒恢复中的作用

摘　　要：目的探讨氟马西尼在不同年龄组妇科腔镜患者全麻术后清醒恢复中的作用。方法 60例全麻下行妇科腔镜手术的患者根据年龄分为Y组(17岁≤年龄≤30岁)和O组(45岁≤年龄≤68岁),每组30例。术中使用咪哒唑仑、芬太尼、苯磺酸阿曲库铵、丙泊酚等药物诱导和维持的气管内插管全身麻醉方式,使BIS值维持在50±5之间。术后均给予氟马西尼(0.01 mg/kg)静注,记录两组患者术毕(T0)及给药后2 min(T1)、10 min(T2)、30 min(T3)、60 min(T4)的BIS值,并记录各组患者恢复自主呼吸至正常时间、拔管时间、需放入口咽通气道个体数。结果①两组患者除年龄外,体重、身高、术中失血量、芬太尼总用量、手术持续时间比较差异无统计学意义(P>0.05)。②组内比较,Y组T1、T2、T3、T4时间点患者的BIS值均显著高于T0时间点,差异有统计学意义(P<0.05),O组T1、T2、T3、T4时间点患者的BIS值均显著高于T0时间点,差异有统计学意义(P<0.05);组间比较,两组患者在T0、T1、T2、T3、T4时间点的BIS值比较差异无统计学意义(P>0.05)。③组间比较,两组患者给药后恢复自主呼吸达标准时间、拔管时间、需放入口咽通气道个体数比较差异均无统计学意义(P>0.05)。结论氟马西尼对不同年龄组患者行妇科腔镜手术全麻后的催醒效果相同,年长患者也可安全使用。Objective To explore the effects of flumazenil on consciousness recovery in differential age gynecological laparoscopic patients post-general anesthesia.Methods Sixty patients undergoing gynecological laparoscopic surgery were divided into group Y（17 years≤age≤30 years,n = 30） and group O（45 years≤age≤68 years,n = 30）randomly,midazolam,fentanyl,atracurium and propofol were used during general anesthesia by endotracheal intubation induction and maintain.Made a stable number of BIS 50 ± 5 during operation.Flumaxenil（0.01 mg /kg） was given to the two groups after post-operation in PACU,BIS value was recorded before（T0） and after the administration of 2 min（T1）,10 min（T2）,30 min（T3）,60 min（T4）,meanwhile,the spontaneous breathing recovery time,extubation time and airway need helped numbers were recorded.Results ①There was no significant difference in two groups of patients with weight,height,intraoperative blood loss,the total amount of fentunyl,operation time except age;②Each of the BIS value in T1,T2,T3,T4 was significantly increased compared with T0 in group Y（P 〈0.05）;the same as group Y,each of the BIS value of T1,T2,T3,T4 were significantly increased compared with T0 in group O;and the BIS value contrast had no significant difference in comparison between the two groups at each time point in T1,T2,T3,T4（P 〉0.05）;③There was no signifiacnt difference in two groups of patients with spontaneous breathing recovery time,extubation time and airway need helped numbers conrtast（P 〉0.05）.Conclusion The flumazenil shows the same effects on consciousness recovery in diffferential age gynecological laparoscopic patients post-general anesthesia,elderly patients may also be used safely.
