摘　　要：目的:研究痰瘀互结证对高血压前期患者hs-CRP、TNF-α、IL-4、IL-6、ICAM-1水平的影响。方法:共纳入初诊为高血压前期患者133例,按照中医证型分为痰瘀互结证69例,非痰瘀互结证64例,同时纳入30例理想血压健康体检者30例作为对照组,测定其血清hs-CRP、TNF-α、IL-4、IL-6、ICAM-1水平,观察高血压前期与上述指标的相关性,并探讨痰瘀互结证对高血压前期患者炎症反应的影响。结果:①高血压前期组与对照组相比,hs-CRP、TNF-α、IL-4、IL-6、ICAM-1水平显著升高(P<0.01);②痰瘀互结组与非痰瘀互结组相比,hs-CRP、TNF-α、IL-4、ICAM-1水平显著升高(P<0.01)。结论:①作为高血压病的早期阶段,高血压前期就已经发生了炎症损伤,进而表明,炎症损伤贯穿了高血压病发生发展的整个过程,是高血压病发生发展的重要环节;②痰瘀互结组患者的炎症因子及hsCRP水平明显高于非痰瘀互结组,说明痰瘀互结组较非痰瘀互结组发生更明显的炎症损伤,为高血压前期中医辨证分型提供客观依据,并为中医药干预高血压前期提供研究方向。Objective ： To study the effect of phlegm and blood stasis syndrome on the levels of hs - CRP, TNF-α, IL - 4, IL - 6 and ICAM - 1 in patients with pre - hypertension. Methods ： 133 cases were included in the first diagnosis of pre - hypertension and according to the TCM syndrome types were divided into phlegm and blood stasis syndrome （69 cases） and no phlegm and blood stasis syndrome （64 cases）. 30 cases with ideal blood pressure were included as control group at the same time. The levels of serum hs - CRP, TNF -α, IL -4, IL -6 and ICAM - 1 were determined and the relationship between above-mentioned indexes and pre-hypertension was observed. We discussed the influence of phlegm and blood stasis syndrome on the inflammatory reaction of patients with pre-hypertension. Results ： （ 1 ） Compared with control group, the levels of hs - CRP, IL - 4, IL - 6 and ICAM - 1 of pre-hypertension group were significantly increased （P 〈 0. 01 ）. （2） Compared with no phlegm and blood stasis group, the lev- els of hs -CRP, TNF-α, IL- 4 and ICAM -1 of phlegm and blood stasis group were significantly increased （P 〈 0. 01 ）. Conclusion ： （ 1 ） As the early stages of hypertension, pre - hypertension has inflammatory damage, suggesting that inflammatory damage runs through the whole process of the occurrence and development of hypertension. It is an important part of hypertension development. （2） The levels of inflammatory factor and hs - CRP of phlegm and blood stasis group were significantly increased, suggesting that the inflammation of phlegm and blood stasis group is more obvious than no phlegm and blood stasis group and providing objective basis for TCM syndrome differentiation of pre-hypertension and research direction of early TCM intervention to hypertension.
