芳香化浊益肾活血法主治过敏性紫癜性肾炎临床研究

摘　　要：目的：探讨芳香化浊、益肾活血法治疗过敏性紫癜性肾炎（HSPN）的疗效。方法：采用中医为主、中西医结合的方法治疗HSPN 60例,待紫癜明显消退,腹痛、关节痛缓解后逐渐减量停用西药,仅用中药（益肾消癜方加减）治疗至尿常规正常后6~18个月。观察尿常规（潜血、蛋白）、24 h尿蛋白定量、尿微量蛋白的变化。结果：（1）95%（57/60）患者治愈,随访至今未复发。（2）尿蛋白、潜血转阴时,38.33%（23/60）患者24 h尿蛋白定量增高,91.67%（55/60）尿微量蛋白增高,治疗至9个月时全部患者24 h尿蛋白定量均正常,但仍有48.21%（27/56）患者尿微量蛋白增高,继续治疗至12、15、18个月时尿微量蛋白增高者则分别降至26.67%（16/60）,22.50%（9/40）和14.82%（4/27）,但三者间无统计学差异（P〉0.05））。结论：芳香化浊、益肾活血法可有效消除尿蛋白、潜血,逐渐降低24 h尿蛋白排泄量和微量蛋白（微量白蛋白、β2-微球蛋白、视黄醇结合蛋白）,促进肾损害的修复。Objective ： To explore the effects of Fangxiang Huazhuo Yishen Huoxue method treating Henoch - Schonlein purpu- ra nephritis （HSPN）. Methods：60 cases of HSPN were treated by routine method and Chinese medicine Yisheng Xiaodian Formu- la. When these symptoms, such us purpura, bellyache and arthralgia disappeared, the routine method would stop, and the patients were only treated with Chinese medicine. We observed the urine routine,24 h urine protein, urine trace protein, The observation continued 6 - 18 months after urine routine revived normal. Results： （ 1 ）95% （57/60） patients were cured. （2） When urine rou- tine revived normal ,38.33 % （23/60）patients＇ 24 h urine protein and 91.67% （55/60）urine trace protein were higher than nor- mal. The treatment continued till to the 9th month and all 24h urine protein levels were normal, but 48.21% （27/56）patients＇ urine trace protein levels were high,till 12th,15th,18th month,27.78% （16/60） ,22.50% （9/40）, 14.82% （4/27） urine trace protein levels were high. Conclusion：Fangxiang Huazhuo Yishen Huoxue method can effectively eliminate urine protein and occult blood, gradually reduce 24 h urine protein and urine trace protein and it is proved to be the effective treatment of HSPN.
