摘 要:目的 探讨糖尿病人群性别差异与新发脑梗死事件的相关性. 方法 采用前瞻性队列研究方法,以空腹血糖≥7.0 mmol/L或<7.0 mmol/L但已确诊为糖尿病、正在使用降糖药物的8 306例糖尿病人群作为观察队列,随访(48.01±3.14)个月,随访期间每半年收集一次新发脑梗死事件情况.分析糖尿病人群性别差异与新发脑梗死事件的相关性.结果 (1)与女性组相比,男性组收缩压、舒张压水平较高,空腹血糖、HDL-C水平较低,差异有统计学意义(P<0.05).(2)男性组累积发生脑梗死事件率高于女性组(2.9%对2.0%),差异有统计学意义(P<0.05).校正年龄、性别、BMI、SBP、DBP、FPG、TC、TG、LDL-C、HDL-C及吸烟因素,Cox比例风险回归分析表明,男性组发生脑梗死事件的相对危险(RR)为女性组的1.58倍(95%CI:1.05~2.38,P<0.05). 结论 男性是糖尿病人群脑梗死事件的独立危险因素.Objective To explore the relationship between gender differences and first cerebral infarction event among diabetes population. Methods This was a prospective cohort study. 8,306 diabetic subjects who determined fasting plasma glucose (FPG) ~7.0 mmol/L or treated with insulin or hypoglyeemic drugs and FPG 〈 7.0 mmol/L as the observation population and were followed- up for (48.01 ± 3.14) months. During the course, cerebral infarction events were determined every 6 months. We analyzed the correlation between gender differences and first cerebral infarction event in diabetes population. Results (1) Compared with the female group, the levels of systolic blood pressure (SBP) and diastolic blood pressure (DBP) were higher but FPG and high density lipoprotein - cholesterol (HDL - C) were lower in the male group, the differences were statistically signifi- cant (P〈 0.05). (2) The cumulative incidence rate of cerebral infarction event was higher in the male group than in the female group (2.9% vs 2.0% ), with statistically significant differences (P〈0.05). Compared with the female group, after adjusting for age, sex, body mass index (BMI), SBP, DBP, FPG, total cholesterol (TC), triglyceride (TG), low density lipoprotein- cholesterol (LDL-C), HDL-C and smoking, Cox regression analysis showed the relative risk for cerebral infarction events in the male group was 1.58 (95 % CI : 1.05 - 2.38, P 〈 0.05). Conclusions Male is the independent risk factor for the cere- bral infarction events in the diabetic group.
