常规器械经脐单孔腹腔镜输卵管切除术41例分析

摘　　要：目的探讨常规器械经脐单孔腹腔镜输卵管切除术的临床疗效、安全性及其优点。方法对嘉兴学院附属第一医院2011年8月至2012年8月41例输卵管妊娠患者行常规器械经脐单孔腹腔镜输卵管切除术,并与同期45例常规腹腔镜下输卵管切除术比较。结果两组相比,单孔腹腔镜组术后切口美观优于传统腹腔镜组,单孔腹腔镜组手术时间(30.43±5.83)min,术中出血量(8.63±3.79)mL,术后排气时间(28.35±7.41)h,住院时间(5.03±0.72)d。传统腹腔镜组手术时间(21.09±3.82)min,术中出血量(8.23±3.31)mL,术后排气时间(29.85±6.24)h,住院时间(5.24±0.63)d。单孔腹腔镜组手术时间略长于传统腹腔镜组,差异有统计学意义(P<0.05),术中出血量、术后排气时间及住院时间两组差异无统计学意义(P>0.05)。结论经脐单孔腹腔镜输卵管切除术在常规腹腔镜器械下可以安全、可靠地完成,术后腹壁瘢痕不明显,易被患者接受。Objective To investigate the effect,safety and excellence of laparo-endoscopic single-site salpingectomy（LESS） with conventional equipment.Methods Compare between two groups who were hospitalized in the gynecology department of the Jiaxing First Hospital between August 2011 and August 2012 one group （n=41） underwent conventional equipment laparo-endoscopic single-site salpingectomy,and another group（n=45） underwent conventional lapro-endoscopic salpingectomy（CLES）. Results The cosmetic outcomes of LESS were better than that of CLES.The operating time of LESS was （30.43±5.83） min，and the intraoperative blood loss was （8.63±3.79） mL , the recovery time of gastrointestinal function was （5.03±0.72 ）d , the length of stay was （5.03±0.72） d. The operating time of CLES was （21.09±3.82） min,and the intraoperative blood loss was （8.23±3.31） mL, the recovery time of gastrointestinal function was （5.24±0.63） d, the length of stay was （5.24±0.63 ）d.So the operating time of LESS was longer（P〈0.05）,but there was no significant difference in intraoperative blood loss,recovery time of gastrointestinal function and length of stay between two groups （P〉0.05）. Conclusion With better cosmetic outcomes,LESS is safe and feasible with conventional equipment and easier accepted by patients.
