摘 要:目的探讨慢性阻塞性肺疾病患者血管内皮素-1(ET-1)与降钙素基因相关肽(CGRP)检测的临床价值。方法选取86例慢性阻塞性肺疾病患者作为观察组,50例同期健康志愿者为对照组,测定两组ET-1及CGRP水平,并结合慢性阻塞性肺疾病临床分级作比较分析。结果Ⅰ、Ⅱ、Ⅲ级慢性阻塞性肺疾病患者CGRP水平分别为(26.8±10.1)μg/L、(23.5±6.9)μg/L、(21.3±3.5)μg/L,均低于对照组,比较差异有统计学意义(P<0.05);Ⅲ级慢性阻塞性肺疾病患者ET-1水平为(90.2±6.3)μg/L,高于对照组及Ⅰ级慢性阻塞性肺疾病患者(P<0.05);Ⅱ级慢性阻塞性肺疾病患者ET-1水平为(74.6±5.5)μg/L,高于对照组及Ⅰ级慢性阻塞性肺疾病患者(P<0.05);相比于吸氧前,Ⅱ级及Ⅲ级慢性阻塞性肺疾病患者吸氧后CGRP水平[(17.5±5.8)μg/L、(19.8±9.6)μg/L]明显升高,ET-1水平[(61.4±6.3)μg/L、(73.2±5.4)μg/L]明显降低(P<0.05)。Spearman相关分析显示慢性阻塞性肺疾病患者CGRP与ET-1间存在显著负相关(r=-0.754,P<0.05)。结论 ET-1及CGRP水平是判断慢性阻塞性肺疾病患者病情的重要指标,缺氧可能通过影响CGRP与ET-1间动态平衡导致慢性阻塞性肺疾病发生。ObjectiveTo study the chronic obstructive pulmonary disease(COPD) in patients with vascular endothelin-1 (ET-1) and calcitonin gene related peptide (CGRP) and clinical value of detection.Methods86 patients with COPD as the observation group, 50 healthy volunteers as control group, ET-1 and CGRP levels of two groups were detected, COPD clinical classification were comparative analyzed.ResultsⅠ,Ⅱ,ⅢCOPD patients, CGRP levels were (26.8±10.1)μg/L, (23.5±6.9)μg/L, (21.3±3.5)μg/L, were lower than the control group, with significant difference (P〈0.05); The level of ET-1 in patients with gradeⅢCOPD was (90.2±6.3)μg/L, higher than that of the control group and grade COPD patients (P〈0.05); The level of ET-1 in patients with classⅡCOPD (74.6±5.5)μg/L, higher than that of the control group and grade COPD patients (P〈0.05); Comparedto the oxygen, CGRP level [(17.5±5.8)μg/L, (19.8±9.6)μg/L] ofⅡ andⅢ COPD patients after oxygen treatment increased significantly, the level of ET-1 [(61.4±6.3)μg/L, (73.2±5.4)μg/L] was significantly decreased(P〈0.05). Spearman correlation analysis showed that there was significant negative correlation between ET-1 and COPD in patients with CGRP (r=-0.754,P〈0.05).ConclusionET-1 and CGRP levels are important index to judge the condition of patients with COPD, hypoxia may lead to COPD occurs through the effect of dynamic balance of CGRP and ET-1.
