摘　　要：目的：探索爱丁堡产后抑郁量表（Edinburgh postpartum depression scale, EPDS）在广州市城区产妇产后抑郁症（postpartum depression, PPD）初步筛查中的临界值。方法采取分层抽样法,抽取广州市天河区3所医院2013年5月1日—9月30日入院的产妇1428例,于产后第3-7天采用中文版 EPDS 测评,同时采用 Beck 抑郁问卷（BDI）作为参照标准,计算 EPDS 各临界值的灵敏度、特异度及 Youden 指数,绘制受试者工作特征（ROC）曲线,计算曲线下面积。结果以 BDI 为参照标准,调查对象中发生 PPD 293例（20．52%）,其中轻度抑郁163例、中度抑郁98例、重度抑郁32例。经过 ROC 曲线测试,EPDS 用于该地区 PPD 筛查的适宜标准为：10-12分为轻度产后抑郁,13-15分为中度产后抑郁,≥16分为重度产后抑郁。结论建立适合各地区经济生活条件的 PPD 筛查临界值,有助于提高 PPD 筛查和诊断的准确性。Objective To explore the critical value of the Edinburgh postnatal depression scale （EPDS） in primary screening of postpartum depression （PPD） in delivery women in urban areas in Guangzhou. Methods A total of 1428 delivery women in three hospitals from May 1 to September 30, 2013 in Tianhe district in Guangzhou were detected with the Chinese version of EPDS on postpartum 3rd -7th d. At the same time, Beck depression inventory （BDI） was adopted as reference standard. The sensitivity, specificity and Youden indexes of all critical values in EPDS were calculated, and receiver operating curve （ROC） of the subject were drawn up, and then area under curve was also calculated. Results Based on the BDI standard, 293 delivery women had PPD （20. 52% ）, in which 163 women were mild, 98 women were moderate and 32 women were severe. The critical values of EPDS in screening of PPD in urban areas in Guangzhou were as follows 10 - 12 score was mild PPD, 13 - 15 score was moderate PPD and the score higher than 16 was severe PPD by ROC detection. Conclusion It＆#39;s necessary to establish critical values of PPD, which can fit in economic and living condition in every city and to improve the accuracy of the screening and diagnosis of PPD.
