摘 要:目的探讨超声造影对宫颈癌血管生成特征的评估价值。方法宫颈癌组25例及对照组20例均进行超声造影增强显像(CEUS),观察宫颈血流灌注模式及微血管分布特点,并使用QLABROI后处理软件,分析时间-强度曲线(TIC)形态及部分参数。采用免疫组织化学技术检测微血管密度(MVD)。结果 CEUS诊断宫颈癌21例,另4例ⅠA1早期浸润癌未能检出。宫颈癌CEUS表现为增强早期病灶区局限性团状或环状高增强,增强晚期病灶内部先减退,周边部呈稍高增强。宫颈癌TIC形态呈"高尖"、"快进快出"或"快升缓降"型,而正常宫颈呈"慢进慢出"型。恶性病灶较对照组达峰时间(TTP)、增强时间(?T)明显缩短,增强强度(EI)及单位时间强度变化(K)明显增高(P均<0.05)。宫颈癌组MVD显著高于对照组(P<0.05)。结论 CEUS能够提高宫颈癌新生血管的显示能力,为准确评估癌肿血管生成及定量研究具有一定临床价值。Objective To investigate the tumor angiogenic features of cerical cancer by contrast- enhanced ultrasonography (CEUS). Methods The blood perfusion patterns and angiogenesis characteristics were observed in 25 cerical cancer cases, and 20 controls. According to the acoustical time-intensity curve (TIC) form , the perfusion quantified parameters and mode on the cervical carcinoma were analyzed by QLAB-ROI software. The microvessel density (MVD) in both two groups were detected on immunohistochemical techniques. Results Among of 25 cases, 21 cases were correctly diagnosed by CEUS while two carcinoma in situ cases and 4 preinvasive ( I A1) cases were not detected. After administration agent, the enhancement of invasive carcinoma homogeneously or heterogeneously in the arterial phase, earlier than the normal rnyometrium with peripheral enhancement in late phase were found. TIC appeared quick-up and quick-down type or quick-up and slow-down type in the malignant lesions, but slow-up and slow-down type in the myometrium of normal uterus. Comparing with the control group, time to peak(TTP) and the enhanced time (AT) in the malignant lesions were significantly shorter while the enhanced intensity (EI) and unit time strength changes (K) were increased obviously (P 〈 0.05). Conclusions CEUS could obviously improve the degree of virtual tumor angiogensis and quantitive study in carcinoma cervix, and it can be worthy for clinical application.
