摘 要:目的利用淋巴结反应性增生模型,观察两种对比剂(Gd-DTPA和HSA-Gd-DTPA)进行MR淋巴造影效果比较。方法制作新西兰兔腘窝淋巴结反应性增生模型16只,完全随机法分成Gd-DTPA组(A组)和HSA-Gd-DTPA组(B组)。仰卧位固定兔子,轴位和矢状位扫描双侧腘窝淋巴结。平扫序列包括T1WI,脂肪抑制T1WI和T2WI。增强扫描:平扫后一侧后肢趾蹼间隙注射Gd-DTPA和HSA-Gd-DTPA,A组于给药后10 min、30 min和60 min行T1WI和T1WI FS扫描,B组于给药后30 min、1 h、3 h、6 h和12h行T1WI和T1WI FS扫描。相关参数与平扫时相同。测量增强前后两组淋巴结的信号强度并计算其强化率。采用配对t检验和两独立样本t检验分析增强效果及其组间差异。结果 (1)经趾蹼间隙给药后,Gd-DTPA后10 min即进入血循环,胫前静脉显影,尿液快速明显强化,淋巴结不均匀强化,而HSA-Gd-DTPA不进入血循环,淋巴管和淋巴结明显、均匀的强化;(2)A组淋巴结的最大强化率(59.9﹪)低于B组的最大强化率(147.2﹪)。结论对比剂的分子量显著影响间隙法MR淋巴造影的强化率和强化过程。Objective To compare the diagnostic value of magnetic resonance (MR) lymphography after subcutaneous injection of two contrast agents, Gd-DTPA and HSA-Gd-DTPA, with hyperplastic lymph node models. Methods Sixteen New-Zealand rabbits with hyperplastic popliteal lymph nodes were made and randomized into Gd-DTPA and HSA-Gd-DTPA groups with subcutaneous (interdigital skin fold) injection of the two contrast agents (0.1 gmol Gd3+) for MR lymphography of the popliteal lymph nodes examined in the axial and saggital orientation. Tl-weighted, T1-weighted fat suppressed, and T2-weighted spin-echo (SE) images of the lymph nodes were obtained in plain scans. The post-contrast scanning started at 30 min, 1 h and 3 h after Gd- DTPA administration and at 10 min, 30 min and 60 min after HSA-Gd-DTPA injection to obtain T1-weighted images with identical imaging parameters. The signal intensity of popliteal lymph node was measured and the enhancement rates were calculated. Results After subcutaneous injection, Gd-DTPA quickly entered blood circulation resulting in obvious enhancement of the anterior-tibial vein and the urine and also in heterogeneous enhancement of the popliteal lymph nodes. HSA-Gd-DTPA didn't enter the blood, causing obvious homogeneous enhancement of the lymphatic vessels and lymph nodes. HSA-Gd-DTPA resulted in higher enhancement rate than Gd-DTPA did. Conclusion The molecular weight of contrast agent has significant influence on the enhancing rate and process in MR lymphography after subcutaneous administration.
