带蒂皮瓣治疗手外伤软组织缺损的临床效果观察

摘　　要：目的探讨腹部带蒂皮瓣在手外伤伴软组织缺损应用中的临床应用效果。方法对收治的37例(76指)手外伤伴软组织缺损伴肌腱、骨外露应用微型腹部带蒂皮瓣进行修复。结果 90指皮瓣全部成活,8指皮瓣边缘出现水疱、瘀紫癍,间断拆除部分缝线,皮瓣边缘血运恢复正常,5指发生皮瓣边缘坏死,经换药自行愈合。结论腹部带真皮下血管网带蒂皮瓣是修复手外伤伴软组织缺损的有效方法之一,手术操作简单,风险小,皮辩成活率高,手术效果比较好,适合临床推广。Objective To investigate the abdominal flap with the clinical effect of soft tissue defect in the hand injury. Methods 37 cases（76 fingers）treated by hand injury with soft tissue defect with tendon, bone exposure using micro pedicled abdominal flap. Results 90 fingers were survived, 8 fingers flap edge blisters, stasis purple macula, removal of part of the flap edge interrupted suture, blood return to normal, 5 finger flap edge necrosis occurred, the self healing dressing. Conclusion Abdominal subdermal vascular network skin flap is one of the effective method of soft tissue defect with repair of hand trauma, simple operation, low risk, skin flap survival rate is high, operation effect is good, suitable for clinical application.
