摘　　要：目的观察游离髂骨联合腹股沟皮瓣移植修复手指背侧骨皮缺损的临床效果。方法自2008年2月至2011年12月我中心收治的7例手指背侧骨皮缺损的患者，手术方案为：3例污染较轻者经急诊清创后I期给予修复，4例感染者I期彻底清除病灶，控制感染后，11期行游离髂骨植骨微型接骨板内固定手指于功能位，腹股沟皮瓣修复软组织缺损。结果术后3—4周断蒂，皮瓣全部成活；临床植骨愈合时间为8周3例，9周4例，平均8．5周；术后3个月复查全部病例均达到骨性愈合。随访10-36个月，皮瓣质地良好，外观满意，髂骨塑形良好，未出现骨髓炎、骨不愈合等情况。结论游离髂骨联合腹股沟皮瓣移植修复手指骨皮缺损是一种理想方法，简单易行，值得临床推广。Objective To observe the flee iliac and groin flap transplantation to repair fmger dorsal skin defects of clinical effect. Methods From February 2008 to December 2011, the center of 7 cases of finger dorsal skin defects of patients, operation scheme is： 3 cases of pollution is light person after emergency debridement in stage I to repair, 4 cases of patients with stage I thoroughly remove lesions, infection control, phase Ⅱ line free iliac bone graft tiny finger bone plate internal fixation in function, groin skin flap to repair soft tissue defect. Results after 3-4 weeks off, the flaps all survived, clinical bone graft healing time of 8 weeks 3 cases, 4 cases were 9 weeks, an average of 8.5 weeks; After 3 months review all cases achieved bony healing. 10-36 months follow-up, flap quality of a material is good, the satisfactory appearance, iliac crest shape is good, does not appear osteomyelitis, bone healing, and so on and so forth. Conclusion Free iliac and groin flap transplantation finger bone skin repair is an ideal method, simple, worth clinical promotion.
