摘 要:目的探讨内镜下建立猪急性重症胆管炎模型的方法。方法 20只猪随机分为模型组和对照组各10只,2组均行内镜下逆行胆管造影,模型组向胆总管内注射大肠杆菌,对照组向胆总管内注射生理盐水,分别于术前及术后24、48、72h检测血白细胞计数及分类、血清谷丙转氨酶、碱性磷酸酶、胆红素、肿瘤坏死因子-α、白细胞介素-1及内毒素水平,观察2组胆管组织病理学改变。结果术后24、48、72h模型组白细胞计数及中性粒细胞比率、血清谷丙转氨酶、碱性磷酸酶、胆红素、肿瘤坏死因子-α、白细胞介素-1及内毒素水平均高于术前和对照组,且随时间延长逐步升高(P<0.05),术前2组上述指标水平比较差异无统计学意义(P>0.05);模型组胆管组织病理学表现为肝脏肿大、淤血,胆管呈急性炎症表现。结论内镜下建立猪急性重症胆管炎模型简单、微创,可用于急性重症胆管炎的实验研究。Objective To explore the method of establishing pig models of acute cholangitis of severe type under endoscope. Methods Twenty pigs were randomly divided into model group and control group, with 10 pigs in each group. Two groups underwent endoscopic retrograde cholangiopancreatography, model group was injected E. Coli and control group was injected normal saline via common bile duct. The changes of peripheral white blood count (WBC), glutamic-pyruvic transaminase (GPT), alkaline phosphatase (ALP), total bilirubin (TB), tumor necrosis factor-a (TNF-a), interleukin-1 and endotoxin were detected before operation, and at the time points of 24, 48 and 72 hours after operation. The pathological changes of bile duct were observed. Results The levels of peripheral WBC, GPT, ALP, TB, TNF-a, IL-1 and endotoxin showed no significant differences between two groups before operation (P〉 0.05), and increased significantly in model group after operation in comparison with before operation and in control group after operation (P〈0.05), and increased gradually along with the prolong of time (P〈0.05). Histopathological examinations showed hepatomegaly, liver congestion and acute biliary inflammatory in model group. Conclusions To establish pig model of acute cholangitis of severe type under endoscope is simple and minimally invasive, and can be used in the related experimental study.
