摘　　要：目的探讨骨髓增殖性肿瘤(MPNs)患者外周血中CD34+细胞水平及临床意义。方法 101例MPNs患者中,慢性粒细胞白血病54例(A组),特发性骨髓纤维化18例(B组),真性红细胞增多症18例(D组)和原发性血小板增多症11例(E组)。A组中慢性期30例(A1组)、加速期11例(A2组)和急变期13例(A3组)。正常健康人28名作为对照(C组)。流式细胞术检测各组患者外周血CD34+细胞百分比、绝对数及单核细胞数。结果 A、B组患者CD34+细胞百分比、绝对数及单核细胞数均高于C组(P<0.01或P<0.05)。与A1组比较,A2、A3组CD34+细胞百分比增高,而单核细胞数降低(P<0.05)。A3组CD34+细胞百分比高于A2组(P<0.05)。结论 CD34+细胞水平有助于MPNs鉴别诊断和病情演变预测。Objective To explore the level of CD34^＋cells in peripheral blood of patients with myeloproliferative neoplasms（MPNs）and its clinical significance.Methods A total of 101patients with MPNs was divided into groups of A（chronic myeloid leukaemia,54cases）,B（idiopathic myelofibrosis,18cases）,D（polycythaemia vera,18cases）and E（primary thrombocytosis,11cases）.Twenty-eight healthy persons were taken as the controls（group C）.Group A was subdivided into groups of A1（in chronic phase,30cases）,A2（in accelerated phase,11cases）and A3（in blast phase,13cases）.The percentage and absolute number of CD34^＋cells and the number of monocytes in peripheral blood were detected by flow cytometry.Results The percentage and absolute number of CD34^＋cells and the number of monocytes in groups of A and B were higher than those in group C（P〈0.01or P〈0.05）.Compared with group A1,the percentage of CD34^＋cells was higher,but the number of monocytes was lower in groups of A2and A3（P〈0.05）.The percentage of CD34^＋cells in group A3was higher than that in group A2（P〈0.05）.Conclusion Detection of CD34^＋cells is helpful in differential diagnosis of MPNs and prediction of the prognosis of patients with MPNs.
