摘 要:目的:比较FLAIR、增强T1WI及增强FLAIR三种序列对不同柔脑膜病变的诊断价值。方法:搜集82例均行FLAIR、增强T1WI和增强FLAIR检查的柔脑膜病变患者的MR图像和资料,分析柔脑膜病变在不同序列上的MRI表现,对比三种序列对病变检出的敏感性。结果:柔脑膜病变呈不同程度的异常脑回样或线样高信号,82例中对病变的检出率分别为:FLAIR 14.63%(12/82)、增强T1WI 19.51%(16/82)、增强FLAIR 87.80%(72/82),三种序列之间差异有统计学意义(c2=101.290,P<0.03)。结论:FLAIR、增强T1WI及增强FLAIR三种序列对不同柔脑膜病变均有不同程度的检出率,其中增强FLAIR对柔脑膜病变的检出率明显高于其他两者,对柔脑膜病变更具有诊断价值。Purpose:To evaluate the diagnostic value of contrast-enhanced T1WI,fluid-attenuated inversion recovery (FLAIR),and contrast-enhanced FLAIR sequences in the detection of leptomeningeal lesions.Methods:Eighty-two patients with different kinds of leptomeningeal diseases confirmed by clinical and pathological results were chosen and examined.MRI protocols included contrast-enhanced T1WI,FLAIR and contrast-enhanced FLAIR sequences.The comparison among these protocols was made to determine which was the best in depicting the lesions.Results:Leptomeningeal lesions were shown as either diffuse or local abnormal hyper-intensity along sulci or cistern on these three sequences.The lesions were shown on contrast-enhanced T1WI in 16 cases (19.51%),on FLAIR in 12 cases (14.63%) and on contrast-enhanced FLAIR in 72 cases (87.80%).The statistical analysis result showed that there was significant difference among them (c2=101.290,P< 0.03).Conclusion:Contrast-enhanced FLAIR imaging is superior to FLAIR and contrast-enhanced TiWI in detection of leptomeningeal lesions.
