摘 要:目的:探讨高频超声在腕管综合征诊断中的应用价值。方法:共42例确诊为腕管综合征而行神经减压术的患者,通过与术前肌电图的对比分析,以手术结果为最终诊断标准,评价高频超声在腕管综合征诊断中的价值。结果:高频超声测量患肢组腕部正中神经横截面面积为0.161±0.049cm2,对照组腕部正中神经横截面面积为为0.081±0.017cm2,两组比较有显著统计学差异,P<0.001。高频超声和肌电图对腕管综合征的诊断准确性分别为92.9%和87.2%,两者比较无统计学差异,P>0.05。与肌电图比较,高频超声还能提示造成正中神经卡压的原因和正中神经受卡压的程度。结论:高频超声在腕管综合征的诊断上具有很高的准确性,在辅助临床选择治疗方案上具有很高的应用价值。Purpose:To evaluate the value of high-frequency ultrasound in the diagnosis of carpal tunnel syndrome.Methods:Forty-two patients who had taken nerve decompression surgery because of clinically confirmed carpal tunnel syndrome were enrolled.The value of high-frequency ultrasonography was evaluated by compared their results with preoperative EMG.Results:The cross-sectional area of median nerve of the patient group and the control group was 0.161±0.049cm2 and 0.081 ± 0.017cm2.There were significant differences between that of patients and controls.The diagnostic accuracy of carpal tunnel syndrome by EMG and high-frequency ultrasonography was 92.9% and 87.2%,there was no significant difference between them.Compared with EMG,high-frequency ultrasound could prompt the cause and degree of median nerve entrapment.Conclusion:High-frequency ultrasound has high accuracy in the diagnosis of carpal tunnel syndrome and high application value for clinical treatment.
