特异性肝胆磁共振对比剂Gd-EOB-DTPA在胆道成像的临床应用及研究

摘　　要：Gd-EOB-DTPA(Primovist?)又名钆塞酸二钠(中文商品名普美显,拜耳医药保健有限公司)是一种新型肝细胞特异性MRI对比剂。其除了具有非特异性细胞外对比剂的性质,在体内还可以特异性地被肝细胞所摄取,并可通过胆汁排泄。由于Gd-EOB-DTPA的独特代谢特点,其肝脏增强MRI检查对显示肝内外胆道系统的解剖结构与通畅情况等可提供较多动态信息,对胆道系统疾病的诊断更具有明显的优势,而国内相关中文文献对Gd-EOB-DTPA应用于磁共振胆道成像上的报道较少,故本文中将对其基本特性及应用原理、胆道成像的应用方法、正常胆道系统成像特点、胆道疾病上的临床应用价值及前景予以综述。Gd-EOB-DTPA （Primovist（R））,which is also called Gadoxetic Acid Disodium（Bayer HealthCare）,is a new kind of hepatocellular-specific MR contrast agents.In addition to its non-specific nature of an extracellular contrast agent,it can be uptaken by hepatocytes specifically and excreted into the bile.Because of the unique metabolic characteristic of Gd-EOB-DTPA,the liver-enhanced MR examination could not only provide more dynamic information for showing the anatomical structure and unobstructed condition of the intra-or extra-hepatic biliary system,but also have more obvious advantages for diagnosis of the biliary diseases.However,there are few related Chinese reports of Gd-EOB-DTPA used in MR cholangiography,we will review its basic characteristics,application fundamentals,application method of biliary imaging,normal biliary imaging characteristics,clinical application value of biliary diseases and prospect in this article.
