胎心监护异常图形与妊娠高危因素的相关性分析

摘　　要：目的探讨胎儿电子监护异常图形与妊娠相关高危因素的关系。方法用地诺前列酮治疗的孕妇244例,临产后记录其胎心监护图形,其中出现异常胎心监护图形者109例为观察组,正常胎心监护图形135例为对照组,分析各种异常图形与妊娠高危因素的关系。结果异常胎心监护图形发生率为44.7%,其中妊娠合并糖尿病、妊娠≥41周伴脐带缠绕、羊水过少是发生异常胎心监护图形的独立危险因素(P<0.05),异常胎心监护图形发生率最高前3位为轻度变异减速、重度变异减速、晚期减速。羊水过少是重度变异减速的危险因素(P<0.05)。妊娠合并糖尿病是产生晚期减速的危险因素(P<0.05)。结论高危妊娠临产后出现异常胎心监护图形的比例较高,糖尿病合并妊娠与晚期减速、羊水过少与重度变异减速有密切关系。Objective To explore the relationship between abnormal electronic fetal heart rate monitoring graphics and pregnancy-related risk factors .Methods Two hundred and forty-four pregnant women treated with dinoprostone were measured for fetal heart rate monitoring graphics in labor ,109 cases of abnormal fetal heart rate monitoring graphics as observation group and 135 cases of normal fetal heart rate monitoring graphics as control group .The relationship between different abnormal graphics and pregnancy-related risk factors was analyzed .Results The incidence of abnormal fetal heart rate monitoring graphics was 44 .7%.Gestational diabetes , pregnancy time≥41 weeks with the umbilical cord wrapped and oligohydramnios were independent risk factors for the occurrence of abnormal fetal heart rate monitoring graphics（P〈0.05）.The highest incidence of abnormal fetal heart rate monitoring occurred in mild variable deceleration ,severe variable decelerations and late decelerations .Oligohydramnios was a risk factor for severe variable decelerations（P〈0.05）,and gestational diabetes was a risk factor for late deceleration （P〈0.05）.Conclusion A higher proportion of abnormal fetal heart rate monitoring graphics occurs in labor of high -risk pregnancy .There is a close relationship between gestational diabetes and late decelerations as well as oligohydramnios and severe variable deceleration .
