前列腺钬激光整块剜除术与电切术安全性及疗效的临床研究

摘　　要：目的比较钬激光整块剜除术与电切术治疗良性前列腺增生(BPH)的安全性与疗效。方法 211例良性前列腺增生患者分为钬激光和电切组二组,钬激光整块剜除术治疗前列腺增生患者109例,前列腺电切术102例。比较二组患者手术前的一般情况,监测和记录患者前列腺剜除时间及粉碎时间,术中及术后失血量,拔管时间等,术后3、6个月复查指标,最大尿流率(Qmx)、国际前列腺症状评分(IPSS)、生活质量评分(QOL)、残余尿量、前列腺特异性抗原(PSA)等并进行统计学分析。结果钬激光手术组在术中出血量、前列腺切除重量、导尿管拔出时间、住院时间等二组比较差异有统计学意义(P<0.01)。术后3、6个月的随访结果,血PSA明显降低,优于前列腺电切组(P<0.01)。IPSS、Qmax、IPSS、QOL、PSA都有明显改善。结论对于良性前列腺增生患者,钬激光整块剜除法是一种安全有效的手术方法,与前列腺电切术相比,手术安全性更好,效果更佳。Objective To observe the safety and curative effect of the holmium laser pluck the whole divi- sion and transurethral resection of the prostate to benign prostatic hyperplasia （BPH）. Methods Two hundred and eleven cases with benign prostatic hyperplasia were divided into holmium laser group and tran- surethral resection group, 109 cases were in treatment of holmium laser enucleation and 102 cases were in treatment of transurethral resection. General situation of the two groups were compared before operation, then monitored and recorded in patients with prostate enucleation time and crushing time, blood loss of in- traoperative and postoperative, time to extubation; meanwhile, the index surgery after 3 months, 6 months, maximal urinary flow rate {Qmx）, International Prostate Symptom Score {IPSS）, quality of life score {QOL）, residual urine volume, prostate specific antigen （PSA） were also analyzed. Results Holmi- um laser operation group was statistically significant difference to transuretbral resection group in the a- mount of bleeding, the weight of resected prostate tissue, the time of catheter pulled out, hospitalization time （P〈0.01）. The follow- up results of 3 months and 6 months after operation, blood PSA was signifi- cantly lower and superior to transuretbral resection group {P〈0.01）. IPSS, Qmax, IPSS, QOL, PSA were obviously improved. Conclusion For patients with benign prostatic hyperplasia, holmium laser enu- cleation block is a safe and effective operation method, compared with transurethral resection of the pros- tate, operation safety and better effect.
