乳腺癌长期生存者心理行为特征与护理方式的相关性

摘　　要：目的探讨乳腺癌长期生存者心理行为特征及和应对方式的相关性。方法采取整群随机抽样方法抽取北京市4家和唐山市4家三级甲等医院内就诊的209例乳腺癌患者,应用T型心理量表和癌症应对问卷进行调查。结果乳腺癌长期存活组T型心理量表的"个性特征(2.12±0.34)分"、"情感表现(2.23±0.24)分、"和"认知习惯(1.73±0.43)分"3个维度的得分低于癌症对照组[(2.34±0.45)分、(2.46±0.31)分、(1.98±0.38)分],(P<0.05);乳腺癌长期存活组在应对问卷的"面对(3.12±0.41)分"和"发泄[(2.70±0.39)分]"2个维度的得分高于癌症对照组[(2.22±0.33)分,(2.58±0.52)分],(P<0.05),在"幻想(1.29±0.33)分"和"屈服(1.95±0.57)分"2个维度的得分低于癌症对照组[(1.56±0.64)分、(2.28±0.62)分],(P<0.05);乳腺癌长期存活者T型心理的"个性特征"、"认知习惯"、"情感表现"和"潜意识"与应对方式的"屈服"、"幻想"和"回避与压力"成正相关(P<0.05),与"面对"和"发泄"成负相关(P<0.05)。结论乳腺癌长期存活者T型心理较癌症对照组不明显,乳腺癌长期存活者多采用积极应对方式,T型心理越不明显越易选择积极地应对方式。Objective To explore the relationship between psychological and behavioral characteristics and coping style in long- term breast cancer survivors. Methods An investigation was conduced using T scale and coping with cancer in 209 breast cancer patients collected using the cluster random sampling method from 8 hospitals with third - level grade- A, four from Beijing and another four from Tangshan. Results The score of ＂personality traits＂, ＂emotional expression＂ and ＂cognitive habit＂ of the T- psychological scale in the long- term survivors of breast cancer were 2.12 ± 0.34, 2.23 ± 0.24, 1.73 ± 0.43, in the control group were 2.34 ± 0.45, 2.46 ± 0.31, 1.98 ± 0.38, the scored in former was lower than in lat- ter {P〈0.05）. In the long- term breast cancer survival group, the ＂face＂ and ＂vent＂ in the cancer Coping Questionnaire was respectively 3.12 ± 0.41, 2.70 ± 0.39 , in the control group was 2.22 ± 0.33, 2.58 ± 0.52, the scored in former was higher than in latter C P〈0.05）. The ＂fantasy＂ and ＂surrender＂ scored respectively 1.29 ± 0.33, 1.95 ± 0.57, the control group scored 1.56 ± 0.64, 2.28 ± 0.62, long- term breast cancer survival group scored lower than the control group （P〈0.05）. In the long- term sur- vivors of breast cancer, the ＂personality traits＂ , ＂cognitive habits＂, ＂emotional expression＂ and ＂subcon-scious＂ , elusion group. of T- mental and the ＂face＂ and ＂vent＂ of the copying style were negatively correlated （P〈0.05） and the ＂yield＂, ＂fantasy＂ and ＂avoidance and stress＂ were positively correlated [P〈0.05）.
