数字钡餐造影在不同病理食管癌放疗后疗效评估中的应用

摘　　要：目的:通过数字钡餐造影检查,探讨不同病理食管癌放疗前后的影像学变化,并分析数字钡餐造影在不同病理食管癌放疗后疗效评估中的作用。方法:收集我院采用单纯放疗方法治疗的食管癌50例,其中鳞癌45例,腺癌4例,鳞腺癌1例,全组患者放疗前后均行数字钡餐造影检查,对比分析其放疗前后的影像表现。结果:放疗后其X线影像学表现出现差异,如病变范围缩短,充盈缺损缩小甚至消失、黏膜皱襞恢复变规整、管腔狭窄程度改善、食管动力学等,其中鳞癌放疗前后的影像表现差异最大,病灶有明显改善,腺癌及鳞腺癌放疗前后的影像表现差异不明显。结论:不同病理食管癌放疗后效果有明显不同,其中鳞癌放疗效果显著,腺癌及鳞腺癌效果较差。研究表明数字钡餐造影对于帮助临床判定不同病理食管癌的放疗效果、及时了解有无并发症及初步估计预后有重要的临床意义。Objective： To investigate the radiologic changes between pre-radiotherapy and post-radiotherapy of different pathological carcinoma of esophagus by digital barium meal imaging. Methods： 50 cases with esophageal cancer were collected,including 45 cases with squamous cell carcinoma,4 cases with adenocarcinoma,1 with adeno squamous carcinoma.All patients took digital barium meal imaging. The imaging findings were compared and analyzed before and after therapy.Results： After radiotherapy,the imaging findings were different,the lesion shortened,filling defect reduced or even disappeared,mucosal fold recovered,the degree of luminal stenosis improved and so on. The imaging differences of squamous cell carcinoma were most significant. The lesions were improved obviously. But the imaging differences of adenocarcinoma and adeno squamous carcinoma were not obviously. Conclusions： The effects after radiotherapy were different among different pathological esophageal cancer. The cases with squamous cell carcinoma got significant effect after radiotherapy,but not for the cases with adenocarcinoma and adeno squamous carcinoma. The study shows digital barium meal imaging can help judge radiotherapy effects on different pathological types of esophagus carcinoma,know timely whether there are complications and preliminarily evaluate the prognosis.
