某院剖宫产围手术期抗菌药物预防应用分析

摘　　要：目的了解剖宫产围手术期抗菌药物使用情况,为抗菌药物合理应用提供参考。方法采用回顾性调查方法,抽取2012年第二季度～2013年第二季度剖宫产手术出院病历150例,对围手术期抗菌药物预防应用情况进行分析评价。结果剖宫产围手术期抗菌药物使用率为100%,均为单一用药。药品选择:使用的抗菌药物为第一代、第二代头孢菌素和头霉素类,共涉及7个品种,其中使用频次占比前3位的抗菌药物依次为头孢硫脒(42.0%)、五水头孢唑啉(38.7%)、头孢西丁(8.0%)。预防用药时机:自2012年第三季度起,所有患者均为结扎脐带后静脉给药。预防用药疗程:自2012年第三季度起,除手术部位感染转为治疗的2例外,其余病例预防用药疗程均没有超过48 h。结论自2012年第三季度起,我院剖宫产围手术期抗菌药物预防应用,在药品选择、预防用药时机和疗程等方面总体是合理的。Objective To analyze the application of antibacterial drugs in perioperative period of cesarean section in our hospital so as to provide reference for rational use of antibiotics.Methods We extracted 150 cesarean section cases from March 2012 to May 2013, and analyzed these data about antibacterial drugs in perioperative period of cesarean section by retrospective investigation.Results The rate of prophylactic application of antimicrobial drugs around caesarean operation was 100.00%; all cases use a single drug. Drug selection： The kind of antimicrobial drugs involved seven species which is the first generation, second generation cephalosporins and cephamycins. The top three antibiotics cefathiamidine, cefazolin sodium pentahydrate and cefoxitin shipped 42.0%,38.7% and 8.0%.Prophylaxis timing： Since the third quarter of 2012,all patients were administered intravenously after ligation of the umbilical cord. Prophylaxis treatment： Since the third quarter of 2012,in addition to surgical site infection treatment into two exceptions,the remaining cases prophylaxis treatment did not exceed 48 hours.Conclusion The application of antibiotics drugs in perioperative period of cesarean section in our hospital was basically reasonable, including the drug selection, the timing of prophylaxis and the application courses.
