两种方法诊断贵阳市某城区20～45岁人群代谢综合征的一致性

摘　　要：目的：了解贵阳市某城区20至45岁成人代谢综合征（Ms）的患病情况，并对2005年国际糖尿病联盟（IDF）标准和全球共识（JIS）标准的诊断结果的进行比较。方法：采用分层整群抽样方法对贵阳市某城区20岁～45岁常住居民共796人（男341人，女455人）进行问卷调查、体格检查及相关代谢指标的测定，采用IDF和JIS标准判断不同性别青年人MS及其各组分的患病率，观察两种标准诊断不同性别青年人MS患病情况的一致性。结果：该年龄段人群用IDF标准诊断Ms的粗患病率为22．2％（男性24．0％，女性19．3％），男女无差异（r=2．57，P〉0．05），用JIs标准诊断Ms的粗患病率为31．3％（男性41．6％，女性23．5％），男性高于女性（，=29．791，P〈0．05）；IDF诊断中心性肥胖为30．5％，JIS诊断中心性肥胖为38．4％；JIS诊断甘油三酯升高、高密度脂蛋白胆固醇降低、血压升高及空腹血糖升高的粗患病率分别为31．3％、55．0％、18．3％、46．4％，二种标准对女性代谢综合征的诊断一致性较好（K=0．876），对总体人群和男性一致性一般（K值分别为0．615和0．747）。结论：IDF和JIS诊断标准均显示该城区青年MS的患病率较高，两种方法对女性MS诊断的一致性较好。Objective： To investigate the prevalence of metabolic syndrome （ MS ） among adults from 20 to 45years old living in certain district of Guiyang, and carry out results comparison according to the standard of 2005 the international diabetes federation （IDF） and standard of joint interim state- ment （JIS）. Methods： A representative sample of 796 adults （341 male and 455 female）, aged from 20 to 45 years old, from certain community in Guiyang were selected by stratified cluster sampling method. Participants were asked to complete questionnaire, physical examination and test related meta- bolic index. IDF standard and JIS standard were adopted to measure MS of different genders and preva- lence rate of different groups, and observing consistency of diagnosis results. Results： The crude prev- alence rate of total MS diagnosed by IDF standard were 22.2% （24.0% males and 19.3% females）, no gender differences （X2 =2.57 ,P 〉0.05）, and 31.3% by JIS standard （41.6% males and 23.5% females）, results from males were higher than that of females （X2 = 29. 791, P 〈 0. 05 ） . Central obesi- ty by IDF standard was 30.5% and 38.4% by JIS standard. JIS standard also detected increased tri-glycerides, lowered high-density lipoprotein cholesterol, increased blood pressure, and increased fast- ing plasma glucose, crude prevalence rate were 31.3%, 55, 0%, 18.3% and 46.4% respectively. The MS diagnostic standard consistency of IDF and JIS works better on females （ K = 0. 876）, works mediocre on general public and males （K=0. 615 and 0.747）. Conclusions： MS is highly prevalent in young adults by both standards in this community, and both standard diagnose MS in females pa- tients with better consistency.
