上半规管良性阵发性位置性眩晕诱发试验的研究

摘　　要：目的探讨运用自主研制的SRM-IV良性阵发性位置性眩晕(benign paroxysmal positional ve r t igo,BPPV)诊疗系统在上半规管BPPV的诱发试验中的临床应用价值。方法本研究中将367例高度怀疑BPPV患者随机分成两组,一组181例依序进行DixHallpike试验及SR M-IV BPPV诊疗系统采用后半规管BPPV诱发试验和上半规管BPPV诱发试验检查,有20例被确诊为上半规管BPPV;另一组186例检查顺序相反,共有21例被确诊为上半规管BPPV,观察并记录该41例确诊为上半规管BPPV的每位患者每种体位下的特异性眼震,计算每种检查方法的检出率,并运用SPSS13.0软件进行统计分析。结果 Dix-Hallpike试验中有26(63.4%)例患者诱发出典型垂直向下眼震;SRM-IV后半规管BPPV诱发试验中有28例(68.3%)患者诱发出典型垂直向下眼震;SRM-IV上半规管BPPV中有35例(85.4%)患者诱发出典型垂直向下眼震。Dix-Hallpike试验与SRM-IV后半规管BPPV诱发试验检出率的差异无统计学意义(P>0.05);SRM-IV上半规管BPPV诱发试验对上半规管BPPV检出率高于其余两种方法,差异均有统计学意义(P<0.05)。结论 SRM-IV上半规管BPPV诱发试验对上半规管可产生有效的刺激,对诊断上半规管BPPV敏感性高,此方法应该在临床得到推广。OBJECTIVE To explore the clinical value of SRM-Ⅳ BPPV diagnosis and therapy system in the positioning test for superior semicircular canal ben ig n pa roxysmal posit ional ver t igo（SSC-BPPV）.METHODS In this research,367 cases were randomly divided into 2 groups.The first group of 181 cases were tested with different positioning tests in the sequence of first Dix-hallpike test,then the positioning test for posterior semicircular canal benign paroxysmal positional vertigo（PSC-BPPV） and at last the positioning test for SSCBPPV.The latter two tests were completed using the newly developed SRM-Ⅳ BPPV diagnosis and therapy system.And 20 cases were diagnosed with SSC-BPPV.The other group of 187 cases were tested with the positioning tests in the reversed sequence and 21 cases were diagnosed with SSCBPPV.The special nystagmus of each SSC-BPPV patient under each pose was registered.The detection rate of each positioning test was calculated.And the result was analyzed with SPSS13.0 software.RESULTS 26 individuals（63.4%） presented the typical positional nystagmus during the DixHallpike test;28 individuals（68.3%） presented the typical positional nystagmus during the positioning test for PSCBPPV under the BPPV diagnosis and therapy system;35 individuals（84.6%） presented the typical positional nystagmus during the positioning test for SSC-BPPV.There＇s no significant difference between the detection rate of Dix-Hallpike test and the positioning test for PSCBPPV under the BPPV diagnosis and therapy system（P〈0.05）.The detection rate of the positioning test for SSC-BPPV under the BPPV diagnosis and therapy system was higher than the other two tests and the differences were statistically significant（P〈0.05）.CONCLUSION For SSC-BPPV,the sensitivity of the positioning test for SSC-BPPV under the BPPV diagnosis and therapy system was high,so the positioning test should be recommended for its efficient stimulus to the superior semicircular canal.
