摘 要:目的分析重型β地中海贫血(地贫)儿童体内自然杀伤(NK)细胞活性及受体表达,探讨输血诱导的NK细胞免疫抑制的机制。方法利用多色流式细胞技术,检测20例重型β地贫儿童、20例轻型β地贫儿童和20例健康儿童的外周血NK细胞CD107a(杀伤活性)、活化型受体(NKP30、NKP46、NKG2D)及抑制型受体(NKG2A、CD158a)的表达;使用细胞内细胞因子染色检测NK细胞分泌IFN-γ的水平。结果与轻型β地贫儿童和正常儿童相比,接受长期输血的重型β地贫儿童外周血NK细胞CD107a表达明显下调(P均<0.01),NK细胞抑制性受体NKG2A表达明显上调(P均<0.01),NKP30、NKP46、NKG2D、CD158a及IFN-γ的表达比较差异无统计学意义(P均>0.05)。结论 NK细胞抑制型受体NKG2A在重型β地贫输血诱导的NK细胞免疫抑制中可能发挥重要作用。Objective To analyze the activity and receptor expression of peripheral natural killer (NK) cells in children with β-thalassemia major and investigate underlying mechanism of the transfusion-in- duced immunodepression. Methods Peripheral blood samples were obtained from 20 children with β-thalassemia major, 20 β-thalassemia minor and 20 healthy volunteers. Expression levels of CD107a, activating receptors (NKP30, NKP46 and NKG2D) and inhibitory receptors (NKG2A and CD158a) within circulating NK cells were detected by muhicolor flow cytometry. The level of IFN-γ secreted by NK cells was measured by in- tracellular cytokine staining. Results Compared with β-thalassemia minor and healthy counterparts ( all P 〈 0. 05 ) , the eytotoxieity (CD107a expression) of circulating NK cells was remarkably down-regulated whereas NKG2A expression, as an inhibitory receptor of circulating NK cells, was significantly up-regulated in children with β-thalassemia major (all P 〈0. 05). However, no statistical significance was observed in the expression ofNKP30, NKP46, NKG2D, CD158aandIFN-γ, (allP〉0. 05) amongthree groups. Conclusion NKG2A, as an inhibitory receptor of NK cells, may play a key role in the transfusion-induced immunodepression of NK cells in children with β-thalassemia major.
