应用二维斑点追踪成像技术评价射频消融术对左室射血分数正常的频发室性早搏患者的影响

摘　　要：目的利用二维斑点追踪成像技术评估射频消融术对左室射血分数正常的特发性频发室性早搏(PVCs)患者的临床意义。方法研究纳入40例特发性频发PVC患者。40例患者均选择接受射频消融手术。对40例患者进行基线的普通超声心动图、二维斑点追踪成像技术及24 h动态心电图检查,3个月后行随访检查。结果 40例患者中,32例手术成功,8例失败。手术成功的32例患者术后的左室射血分数、左室舒张末内径及左室收缩末内径较术前均未见显著改变(P>0.05),而左室短轴应变、左室圆轴应变及左室长轴应变均较术前有显著改善(P<0.05);结论经导管射频消融术能够改善频发PVC患者早期隐匿的左室功能损害。Objective Using two-dimensional （2D） speckle tracking imaging technique to evaluate the clinical significance of radiofrequency catheter ablation （RFCA）to frequent premature ventricular contractions （PVCs） patients with normal left ventricular ejection fraction （LVEF）. Methods Total 40 patients who met the criteria were enrolled to this study. All of them received RFCA operation. Traditional echocardiography , two-dimensional speckle tracking echocardiography （2D-STE）and 24-hour Holter were performed before operation and 3 months after operation. The main outcome assessments were the PVC burden , LVEF, left ventricular end systolic diameter （ LVESD）, left ventricular end diastolic diameter （LVEDD）, left ventricular circumferential strain （ LVCS）, left ventricular longitudinal strain （LVLS）, left ventricular radial strain （LVRS）. Results Among 40 patients undergoing RFCA , 32 succeeded ,8 failed. There were no significant changes in LVEF, LVESD, LVEDD in the patients who underwent successful RFCA （P〉0.05）. However, with 2D-STE, a significant improvement of strain values （LVCS, LVLS and LVRS） was seen in all the three axial directions （P 〈0.05）. Conclusions RFCA can improve the subtle LV dysfunction of frequent PVCs patients.
