右美托咪定对七氟醚抑制支气管镜检查气道反应的最低肺泡有效浓度的影响

摘　　要：目的评价右美托咪定对七氟醚抑制支气管镜检查时患者发生气道反应的最低有效呼气末肺泡浓度（MACbar）的影响。方法选择ASAⅠ~Ⅱ级拟行纤支镜检查患者,随机分为七氟醚组（S组）和右美托咪定复合七氟醚组（DS组）。S组和DS组分别静脉输注生理盐水和右美托咪定0.5μg/kg,10 min输注完毕,观察10 min后开始诱导。两组以潮气量法实行七氟醚吸入诱导,患者意识消失后置入喉罩。第一个患者七氟醚的支气管镜检查剂量按照预试验中气道反应出现阴性的剂量设定,按照改良序贯法确定相邻患者开始支气管镜检查时七氟醚呼气末肺泡浓度,相邻浓度之比为1：1.1,用概率单位回归法分别计算出两组七氟醚抑制支气管镜检查发生气道反应的MACbar及其95%CI。结果 S组和DS组抑制支气管镜检查气道反应的最低有效呼气末肺泡浓度（MACbar）分别为3.00%和2.12%,95%CI为2.84%~3.10%和1.99%~2.23%。结论静脉输注右美托咪定0.5μg/kg可降低七氟醚抑制支气管镜检查气道反应的MACbar。[ Objective ] To evaluate the effect of dexmedetomidine on the minimum alveolar concentration （MACbar） and 95% confidence interval （CI） of sevoflurane inhibit airway responsiveness during laryngeal mask airway of fiberbronchoscopy examination. [ Methods ] ASA Ⅰ or Ⅱ patients were randomly divided into sevoflurane group （Group S） and dexmedetomidine combined sevoflurane （Group DS）. Two groups were intravenous infusion normal saline and dexmedetomidine 0.5 μg/kg respectively. It was finished in 10 minites. Every patient undergoing fiberbronchoscopy examination was inducted by the tidal volume method. When the patient＇s consciousness disappears, a laryngeal mask was putted in. intubated.The first patient of the series was tested by the predicted examination. The dose of sevoflurane adjacent patients to check was determined using the modified Dixon＇s up-and-down method. Probability analysis was used for calculating MACbar and 95% confidence interval（CI）. [ Results ] The MACbar of sevoflurane were 3.00% and 2.12% in group S and group DS, and 95% CI were 2.84%-3.10%,1.99%-2.23%. [ Conclusion ] Intravenous infusion dexmedetomidine 0.5 μg/kg can depress the MACbar of sevoflurane inhibit airway responsiveness during laryngeal mask airway of fiberbronchoscopy examination.
