市级统筹与县医院业务量变化的关联探析

摘　　要：目的 :分析医保市级统筹与县医院医疗指标变动的关联,提出医保支付政策调整和增强县医院竞争力建议。方法:采用描述性统计方法,对县医院在市级统筹前后4年的主要医疗指标增长率进行分析。结果 :市级统筹后入院人数、门诊诊疗人次、手术总人次增长率均有一定程度下降,其中手术人数出现负增长。结论 :市级统筹对县医院业务量有影响,但从根本上说还是医疗资源结构失衡,医疗机构分级诊疗缺失。要加大县医院改革和建设力度,增强其竞争力。同时,在市级统筹下要设计合理的支付政策,引导合理就医。Objects：To analyze the influence of health insurance municipal co-ordination on the change of major indicators in county level hospitals, therefore to propose the adjustment of medical insurance payment policy and to strengthen the competitiveness of county hospitals. Methods：The growth rate of the major medical indicators in county hospitals after four years of health insurance municipal co-ordination were analyzed by descriptive statistical methods. Results：After the municipal co-ordination, the growth rate of admission amount, outpatient amount and the total number of surgery exhibited a certain degree of decline and the number of surgery experienced negative growth. Conclusion： The municipal co-ordination had a impact on the volume of services in county hospitals, but it is fundamentally due to the structure imbalance of medical resources and the lack of classification in medical institutions. It is advised to increase the intensity of reform and the construction of county hospitals therefore to improve the competitiveness of county hospitals. Meanwhile, under municipal co-ordination, it must design a reasonable payment policy to guide proper treatment.
