血清中α1-MG、β2-MG、CRP与UA联合检测在结核治疗后早期肾损害评价中的价值

摘　　要：目的:分析短期化疗后肺结核患者血清中α1微球蛋白(α1-MG)、β2微球蛋白(β2-MG)、超敏C反应蛋白(CRP)检测在早期肾损害诊断中的作用。方法:将我院收治的肺结核患者按初治标准方案治疗,对比结核患者与正常人群以及治疗前后患者血清中α1-MG、β2-MG、CRP变化水平。结果:治疗后患者尿酸(UA)、尿素氮(Urea)、α1-MG、β2-MG、CRP浓度与治疗前相比显著升高(P<0.05);而治疗前后患者肌酐(Cr)浓度无明显变化(P>0.05);治疗后患者血清UA、α1-MG、β2-MG、CRP单项检测阳性率显著上升,其中联合检测阳性率显著高于单项检测阳性率(P<0.05)。患者血清UA与Cr无明显相关性(P>0.05),而与α1-MG、β2-MG、CRP水平呈明显正相关(P<0.05)。结论:短期化疗后的结核患者联合检测血清生化指标检测能够早期诊断药物性肾损害,值得临床推广。Objective To investigate the value of combination detection of α1-MG、β2-MG、CRP and UA in diagnosis of early renal damage in tuberculosis patients after short-term treatment. Methods： A total of 120 eases of pulmonary tuberculosis and 90 healthy cases were selected as the control group. Serum protein, blood uric acid （UA）, the concentration of C reaction protein （CRP） levels before and after anti tuberculosis treatment were measured. Results： UA, Urea, a 1-MG, I] 2-MG, CRP concentrations were significantly increased after treatment （P〉0.05） ; and there was no significant change in Cr concentration before and after treatment （P〉0.05）. Before treatment, the positive rate of indexes were decreased after treatment, serum UA, α1-MG、β2-MG、CRP single positive rate were increased significantly, while the positive rate of combined detection o{ the highest （87.50%） was significantly higher than the positive rate of the single test （P〈0.05）. Correlation analysis showed that, there was no significant correlation between serum UA in patients with Cr （P〉0.05）, and was positively correlated with α1-MG、β2-MG、CRP levels （P〈0.05）. Conclusion： The short-term chemotherapy combined with detection of serum α1-MG、β2-MG、CRP levels can provide reliable, sensitive laboratory basis for early diagnosis of drug-induced renal damage, and it is worthy the clinical promotion.
