摘　　要：目的:观察视黄醇结合蛋白(RBP)对2型糖尿病(T2DM)患者早期肾损害的检测情况,分析其检测价值。方法:选取在本院治疗的2型糖尿病患者60例作为研究对象。检测所有患者24h内的尿白蛋白排泄率,根据检测结果分为A组(正常白蛋白尿)、B组(微量白蛋白尿)以及C组(临床白蛋白尿)。再选取同期来本院检查的健康者32例作为对照组。分别对以上研究对象进行体质量指数(BMI)、血糖(FPG)、胰岛素(FINS)、血脂(TG)、低密度脂蛋白(LDL-C)、尿白蛋白排泄率(UAER)、血中的RBP、尿中的RBP以及糖化血红蛋白(HbA1C)等检测。结果:随着UAEG的增加,2型糖尿病患者的血RBP和尿RBP也相应增加,A、B以及C组之间的血RBP和尿RBP比较,差异具有统计学意义(P<0.05),与对照组比较,差异具有统计学意义(P<0.05)。根据血RBP和尿RBP的相关性分析结果显示,与患者的年龄、HbA1C以及LDL-C无关,与UAER、FPG以及HOMA-IR有关。结论:RBP可以作为检测2型糖尿病患者早期肾损害的指标,因此2型糖尿病患者应尽早检测RBP。Objective： To explore the value of RBP for detection of early renal damage in patients with T2DM. Method.. A total of 60 patients with type 2 diabetes were enrolled, and excretion rate of urinary albumin within 24 hours were detected in all patients. And all the patients were divided into group A （normal albuminuria group）, group B （microalbuminuria group） , and group C （Clinical albuminuria group）. Another 32 healthy people who got regular check during the same period were chosen as the control group. The BMI, FPG, FINS, TG, LDL-C, UAER as well as RBP in blood and HbAIC from all patients were detected in all patients. Results： Serum and urine RBP increased with the increase of UAEG showing significant difference among group A, B and C （P〈0.05）. Correlation between Serum RBP and urine RBP showed they were not correlated with patients＇ age, HbA1C as well as LDL-C but they were correlated with UAER.FPG as well as HOMA-IR. Conclusion： RBP can be used for detection of early renal damage in patients with type 2 diabetes. Therefore, RBP detection should be carried out in patients with type 2 diabetes as early as possible.
